Please find attached, for your perusal, two copiet, of CTI, INC's revised
"Radiatien Safety Program" (Revision 19). This revision reflects address
changes per NRC license amendments No. 19 and No. 4. The revision she..ts
area @ed explain the changes made tc the program. Please t.ote that no
changes have been made to the text previously submitted and approved. ,.lso
attached is marked up copies of pages of the present program to illust: ate
changes made.
A final radiation survey was performed at 3512 International Way, Fairb nks,
Alarka on 9-30-38. A visual survey was also cot; ducted to insure that no
exposure devices, source changers, or collimators were left behind.
Please remove the Fairbanks facility from our license.
It is my understanding that no additional amendment fee is required.
Should additional information be needed, please contact the writer.

All radiography performed under the license issued to CTI,INC. will be
performed at temporary job sites in Alaska and at remote job sites in
Agreement and Non-Agreement States under reciprocity of the NRC
license.
2.0
STORACE FAC",ITY
2.1
Pe rmanent storage of radioactive material and devices will be stored
at 4831 Old Seward Hwy., Suite 107, Anchorage, Alaska 99503.
A special storage vault has been constructed at 4831 Old Seward Hwy.,
Anchorage, Alaska 99503 utili:ing sheets of lead to bring the radiation
levels down to less than 2 mrem per hour at the surface.
This storage
vault will not be used for radiography utilizing byproduct materials
(See enclosed drawing).
The devices will be locked and the storage facility will be kept ic ked.
The storage facility will be posted "Caution - Radioactive Materiais"
on the visible sides.
Radiation levels at the surface of the storage facility will be lets
than 2 mrem per hour.
2.2
On remote job sites, the camera vill be stored on the dark rooms of
the transporting vehicles. Each vehicle is equipped with a source
storage container adequately posted and with a locking device.
Radiation levels on transporting vehicles will be kept at 2 mrem or
less when sourses are stored temporarily in them.
2.3
A shield l :torage room has been constructed at 4831 Old Seward Hwy.,
Anchorage Alaska. Inspection of the shielded room will be conducted
at 6 month intervals by the Radiation safety officer per instructionf
of CTI, INC.'S Shielded Room Inspection (Semi-Annual) Form.
Radiation levels at the surface of the shielded room will be less
than 2 mrem per hour.
2.4
T.e radiation Safety Officer will have the keys for controlling
access to permanent storage f acility (See enclosed drawing) .

All records required under the NRC regulations for review and
inspection will be kept in the office of the Radiation Safety
Officer at 4331 Old Seward Hwy. , Ar.cborage. Alaska 99503.

GENERAL POLICY
1.0
The procedures stated herein are designed to control the use
of Radiation Producing devices employed by CTI, INC., 4531
Old Seward Hwy., Anchorage, Alaska 99503,
1.1
Direct responsibility for the overall radiation safety program
in regards to radiographic operations is assigned to Sandy M.
Watson. He is designated Radiatien Safety officer for all
radiographic operations.
,
1.2
Radiographers shall be trained and qualified in accordance
with Section VI, Appendix A of the License Application.
Qualified radiographers shall have the authority to use and
supervise the use of, while in constant attendance at the
site, the sealed sources authorized by the company license.
1.3
A copy of these instructions will be given to each employee
concerned with the handling or using of radiation producing
devices. Other company personnel after training is completed
as required by the company training program, will be designated
as assistant radiographers and may operate the exposure equip-
ment under the personal supervision of a qualified radiographer.
1.4
Each concerned employee will sign a statement as to the fact
that he/she has read and understands these instructiens prior
to per'orming and work with radiation producing equipmenc.
1.5
Radiation Safety Regulations of the United States Nuclear
Regulatory Commission have been used as guides in the preparation
of these instructions.
The objectives of these instructions
presented here are to protect the health of the general public
and employees of the firm, and to prevent disabling accidents.
It is the responsibility of every concerned employee to have
a working knowledge of these instructions.
1.6
The buclear Regulatc ry Ccenist .on and the Agreement States have
the authority to mcdify, suspend or revoke any radioactive material
license which they issue, or to take any action necessary to insure
careful handling of radiation producing equipment in their juris-
diction. These instructions must be followed explicitly as set
forth by Federal and State authorities.
If any doubt exists
concerning these instructiens or the safety conditions in the
performance of radiographic operations, the Radiatien Safety
Of ficer shall be contacted immediately.
1.7
Violations of the conditiens and procedures set forth in these
instructions, by any employee, shall be just cause for punitive
action.

EMERGENCY PROCEDURES

In the event that a source is lost or stolen, immediately notify
the Rcdiation Safety Of ficer or the Radiation Safety Consultant.
If not available, notify the Nuclear Regulatory Commission.
1.5
In the event of a fire, the radiographic exposure device (camera)
in use is to be returned to a safe condition, locked and removed
from the fire hazard area.
If a radiation source cannot be returned
to a safe condition and removed from the area, notify the Fire Depart-
ment personnel when they arrive at the site.
Also notify the
Radiation Safety Officer. Radiation Safety Consultant c- the Nuclear
Regulatory Commission.
In the event of any other form of accident involving radioactive
material or radiation producing devices, take applicable safety
measures and notify the Radiation Safety Of ficer or the Radiation
Safety Consultant.
If the event cannot be resolved, immediately
notily the Nuclear Regulatory Commission.

All radiography performed under the license issued to CTI. Inc. Will be
performed at temporary job sites in Alaska and at remote job sites in
Agreement and Non-Agreement States under reciprocity of the NRC
license.

A special vault has been constructed at 3512 Ineernnein-41

utili:ing sheets of lead to bring the radiation levels down to less
than 2 mrem per hour at the surface.
This storage vault vill not be
used for radiography utilizing byproduct materials (see enclosed drawina).
The devices vill be locked and the storage facility will be kept
locked.
The storage f acility will be posted "Caution - Radioactive
Materials" on the visible sides.
Radiation levels at the surface of the storage facility will be less
than 2 mrem per hour.
2.2
On remote job sites, the camera vill be stored on the dar? rooms of
the transporting vehicles.
Each vehicle is equipped with a Source
Storage Container adequately posted and with a locking devit,.
Radiation levels on transporting vehicles will be kept at 2 mrem or
less when sources are stored temporarily in them.

Inspection of the shielded room vill be conducted at 6 month
intervals by the Radivtion Safety Officer per the instructions of
CTI. Inc.'s Shielded Room Inspection (Semi-Annual) Fo rm .
Radiation levels at the surface of the shielded room vill be less
than 2 mrem per hour.

The Radiation Safety Officer will have the keys for controlling
access to permanent storage facility (see enclosed drawing).

GENERAL POLICY

The procedures stated herein are designed to control the use
of Radiation producing dev;ces employed by CTI. INC. . - 3512 ~
Intemational Way, Fairbanks, Alaska 99701.

1.1
Direct responsibility for the overall radiation safety program
in regards to radiographic operations is assigned to Sandy N.
Watson. He is designated Radiation Safety Officer for all
radiographic operations.
1.2
Radiographers shall be trained and qualified in accordance
with Secticn VI, Appendix A of the License Application.
Qualified radiographers shall have the authority to use and
supervise the use of, while ia constant attendance at the
site, the sesled sources authorized by the company license.
1.3
A copy of there instructions will be given to each employee
concerned with the handling or using of radiation producing
devices. Other company personnel af ter training is completed
as required by the company training program, will be designated
as assistant radiographers and may operate the exposure equip-
ment under the personal supervision of a qualified radiographer.
1.4
Each concerned employee will sign a statement as to the fact
that 'he/she has read and understands those instructions prior
to perforuing and work with radiation producing equipment.
1.5
Radiation Safety Regulations of the United States Nuclear
Regulatory Commission have been used as guides in the preparation
of these instructions. The objectives of these instructions
presented here are to protect the health of the general public
and employees of the firm, and to prevent disabling accidents.
It is the responsibility of every concerned employee to have
a working knowledge of these instructions.
1.6
The Nuclear Regulatory Commission and the Agreement States have
the auth^rity to modify, suspend or revoko any radioactive material
license which they issue, or to take any action necassary to insure
careful handling of radiation producing equipment in their juris-
diction. These instructions must be followed explicitly as set
forth by Federal and State authorities.
If any doubt exists
concerning these instructions or the safety conditions in the
perfomance of radiographic operations, the Radiation Safety
of'icer shall be contacted immediately.
1.7
Violations ci the conditions and procedures set forth in these
ins t ructiont. , by any employee, shall be just cause for punitive
action.

In the event that a source is lost or stolen, immediately notify
the Radiation Safety Of ficer or the Radiation Safety Consultant.
If not available, notify the Nuclear Regulatory Commission.
1.5
In the event of a fire, the radiographic exposure de'tice (camera)
in use is to be returned to a safe condition, locked and removed
from the fire hazard area.
If a radiation source cannot be returned
to a safe condition and removed from the area, notify the Fire Depart-
ment personnel when they arrive at the site.
Also notify the
Radiation Safety Officer, Radiation Safety Consultant or the Nuclear
Regulatory Con 11ssion.
In the event of any other form of accident involving radioactive
material or radiation producing devices, take applicable safety
ceasures and notify the Radiation Safety Officer or the Radiatior
Safety Consultant.
If the event cannot be resolved, iemediatt'.y
notify the Nuclear Regulatory Commission.
