Dear Mr. Davis This letter is in response to the inspection conducted by Messrs.

T.M. Tongue, T.E. Taylor, G.A. VanSickle, and R. Bocanegra on August 28, 1988 through October 15, 1988 of activities at Braidwood Station. Reference (a) indicated that certain activities appeared to be in violation of NRC requirements. The Comonwealth Edison Company response to the Notice of Violation is provided in the enclosure.

ENCLOSURE COMMONWEALTH EDISON COMPANY RESPONSE TO INSPECTICH REPORT NOS. 456/88026; 456/88026 VIOLATION: (456/88026-01; 457/88026-01)

Braidwood Station Technical Specification Section 6.8.1.a requires that written procedures be established, implemented, and maintained to cover applicable activities recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Procedures for abnormal, of f-normal, or alarm conditions are recommended in Appendix A of Regulatory GrIde 1.33. Braldwood Alarm Response Procedure BwAR 1-1-B7, "Secondary Samplin i Panel / Sink Trouble,"

l Revision 52, April 1987, requires that operators immedit tely notify the Radiation Chemistry Department in response to a seconde y sampling panel / sink trouble condition (Alarm No. 1-1-B7).

Contrary to the above requirement, on September 14, 1988, RAD / Chem Department personnel were not notified of a secondary sampling panel / sink trouble condition when indicated by alarms which began about 9:43 p.m. on Unit 1 and remained unaware of the condition until approximately 11 p.m. on September 14, 1988, when a Radiation Chemistry Technician discovered the condition.

RESPONSE

l Commonwealth Edison acknowledges that chemistry personnel were not I notified when the secondary sampling panel / sink trouble annunciator alarmed in I the control room on September 14, 1988 about 9:43 p.m. However, chemistry was notified at the beginning of the shift by the Station Control Room Engineer that the annunciator alarmed and had been in and out of alarm during the previous shift. Chemistry was requested to closely monitor the secondary sample panel. The annunciator continued to periodically alarm and operations personnel in the control room, because of previous chemistry notifiestion, did not notify chemistry every tine the annunciator alarmed. When chemistry l personnel were made aware of the secondary chemistry excursion, immediate l action was taken to properly evaluate the excursion and determine pre Ser I corrective action. Due to the abnormal secondary side chemistry parameters Units 1 and 2 were taken off line at approximately 4:39 a.m. and 5:37 a.m. on September 15, 1988, respectively. The secondary chemlatry excursion had no significance to public health and safety.

. . CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED:

Appropriate shift personnel have been verbally informed, by Braidwood Station upper level management, that Chemistry is to be notified when the secondary sampling panel / sink trouble annunciator 31 arms in accordance with BwAR 1-1-B7, Revision 52.

CORRECTIVE ACTION TAKEN TO AVOID TURTHER VIOLATION:

Braidwood Station General Information Notice 88-27 on adherence to procedures was issued to appropriate Braidwood personnel on October 11, 1988.

This notice emphasizes the importance of adhering to procedures. To further emphasise this issue the General Informations Notice along with the Notice of Violation and the response to it will be included in the Shift Engineer's tailgate sessions which will be concluded by December 31, 1988.

A training program will be developed to address einnunciator, significant event recorder and local alarms. This training will be made available to appropriate operating personnel. The program is orpected to be developed by March 1, 1989 with training expected to be completed by July 1, 1989.

Full compliance is expected to be ,chieved by July 1, 1989.
