A. McNeill, Jr. dated October 15, 1986, attached please find the Bailey 862 Solid State Logic Module ( SS LM ) Bi-Monthly Failure Report for the Hope Creek Generating Station (HCGS).
This report covers both IE and non-lE modules and summarizes inservice failures per month by class, assembly type and failure type.
With respect to the last two month period, October 1987 - November 1987, 11 Bailey 862 SSLM failures were identified that had an affect on the circuit in which they were used.
These failures are discussed in the attached Bi-Monthly Report.

11 Modules Failures - There were 11 SSLM failures which occurred in the two month period beginning October 1, 1987 through November 30, 1987.
Please note that 1 failure which j
occurred in October directly and immediately resulted from l
inadvertent personnel error.
While this failure is included above, it has not been included in the failure rates as it does not represent a random type failure or failure which is related to the design, i
manufacture or application of the module.
Excluding this module from the failure rates will increase the accuracy of the reliability prediction.
