Corrective SteDs which have been taken and the results achieved 4

l Upon NRC notification of this violation, the procedures were formally approved by the Manager, Nuclear and Industrial safety on November 16, 1987.

Runoff and distribution was completed by November 17, 1987.

The NRC was immediately notified of our corrective action on November 17, 1987 Although undocumented, the H.P. procedures which were is use 1

had been concurred in by the H.P. Specialist and the NIS Manager.

Corrective Steps which will be taken to avoid further violationst UNC will respond promptly to inspection observations.

The date when full conD11ance will be achieved:

Item 3 of Details - HEPA Filter Testinq Maintenance work records were immediately checked for each of the ventilation systems in question.

The records, completed at the time of the testing, stated that the DOP test was acceptable.

However, to assure that the systems were functioning as tested, each system was ratested to verify the required efficiency.

Based on this test and an observed differential pressure increase, one system's filters were changed.

All HP technicians have been reinstructed on the necessity for proper documentation of compliance to the 99.95%

efficiency requirement in HEPA filter acceptance testing.

Item 3 of Details v Coacial Work Permit As noted, when additional information is obtained from the SWP originator by phone or direct contact, the H.P.

personnel will add the information to the SWP form.

Vendor review of the factor difference of 100x resulted in correction of the data.

Laboratory data was based on i

analysis of a 10ml sample which was not corrected to the 1 liter reporting basis.

Health Physics personnel have been reminded of their responsibility for prompt review of vendor data, and investigation and resolution of discrepant blind sample I

results.
