Dear Sir:
In accordance with the provisions of 10 CFR 71.95 (A), which requires the reporting of any instance in which there is a significant reduction in the effectiveness of an authorized packaging during use, please be advised of the following:
Description of Incident On March 11, 1988, a spent fuel 61pping cask, nx del number NLI-l/2, serial number 5462-4, overturned while being transported t.-site at a facility being operated for the V. S. Department of Energy.
During the incident the cask impact limiters functioned as intended and were damaged.
Condition of Shipment At the time of the incident, the cask was being transported in an ISO container, configuration D of its license.
The cask was empty and was enroute to be loaded.
Consequences No personal injury and no release of radioactive material resulted from the incident.
The cask remained inside of the ISO container and the trailer remained attached to the tractor.
The trailer, the ISO container and the impact limiters sustained damage, but no visible damage occurred to the cask.
Damage to the front and rear impact limiters of the cask has rendered this cask non-serviceable.
Cause of Incident Based on an inspection at the incident site, interviews conducted with the drivers and the available witness, it has been concluded that the irident was attributed to the unstab'e condition of the system due to the high center of gravity of the load.

GND/88/036/QAD Page 2 Immediate Corrective Action On March 11, 1988, all (five) of the Model NLI-l/2 Shipping Cask Packages were voluntarily removed from service pending a full investigation and Mr.
Charles E.
MacDonald, Chief of Transportation Certification Branch was notified by telecon.
This notification was confirmed in writing on March 14, 1988.
Final corrective action will br: provided to Mr. Charles E.
MacDonald prior to returning the cask to service.
