LOW-LEVEL WASTE MANAGEMENT Workshop on Public Participation in the Development and Siting of LLW Disposal Facilities On November 17-18, 1988 a representative of LLWM attended a Workshop on Public Participation in the Development and Siting of LLW Disposal Facilities. The workshop was 1,01d in Washington, DC by the LLW Forum and included representatives of States and Regional Compacts charged with developing and implementing public participation and education programs on LLW disposal.

Resource persons from outside the LLW area led discussions on new and different approaches l

to public involvement in siting controversial facilities.

State and Compact representatives led sessions on programs at the state and local level, types of informatic'n materials, and management considerations in program development.

A variety of approaches to public participaticn were explored during the two day session, with the consensus being that no one approach will work in all parts of the country and that state and local considerations are critical to the development of such programs.

l Workshop on Organization and Management Research On November 17-18, 1988 Brookhaven National Laboratory conducted a workshop for the Human Factors Branch in Bethesda to develop a research agenda in the l

area of organization and managawant. More than thirty-five (35) people participated in the workshop.

Participants included management and l

organization science experts from academia, private consulting firms, and representatives from the Comission, RES NRR and AE00 who are cognizant of organization and management related work.

BNL will submit a final letter report covering the results of the workshop by December 19, 1988. The results will be used to guide further development of the organization and management element of the Human Factors Research Program Plan, t

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending November 25, 1988 HATCH, UNIT 1 The Unit 1 torus is supported by inner and outer columns at each of the 16 miter joints, plus intermedi.ie supports batween the miter joint supports. As part of the Mark 1 containment long term program, each of these support columns was anchored to its concrete support pad by 6 rock bolts. A later fix included welding heavy steel support pieces to the columns.

Recently, a Region !! inspector observed that one of the rock bolts was bent.

Upon examination, the licensee found that many of the miter joint support colun bolts (192 total) are bent, with deflections ranging up to about 3/4 inch.

The licensee believes that the bolts were bent as a result of thermal stress resulting from the welaing that took place after the columns were anchored.

However, an engineering evaluation is underway and the licensee plans to meet with the staff about November 30, 1988, to present the results of the evaluation.

Unit 1 presently is shutdown for a maintenance / refueling outage, with restart scheduled for early December. Several bent bolts also have been observed on Unit 2 which has a different design for the torus support.

SHOREHAM On November 21, 1988 the Emergency Planning Licensing Board (OL-3) issued a Memorandum and an Order with one dissenting opinion which found:

2 SAFEGUARDS International IAEA Advisory Group Meeting Mr. Breskovic participated in the Advisory Group Meeting on Containment and Surveillance from October 31, 1938 to November 4, 1988 at the IAEA in Vienna, Austria.

Purpose of the meeting was to advise the IAEA on how to establish sufficient assurance of containment and surveillance (C/S) devices. Two papers were aroduced, one on the objective assurance characterization of C/S devices, and tie other on the evaluation of the in-field performance of in-situ verifi-able-seals.

lutomated Publication of SSEL The Division of Safeguards and Transportation, NMSS has begun development of a microcomputer database program to automate preparation of NUREG-CD25 the Safeguards Summary Events List (SSEL). This publication, which is updated and published yearly, is a compilation of threat-related licensee safeguards events.

The SSEL has, previously been prepared as a word processing document which necessitated tediou: ef f ort to accor.nodate minor changes. The new SSEL program, being developed by Dr. Lance Lessler, will treat the event data and text as database records, allowing for sophisticated data search, report preparation, and facilitation of trend analysis as well as automated preparation of the SSEL for publication. The program will result in expanded analysis focus for staff resources.

3 TRANSPORTATION Status of Survey Being Conducted to Determine States / Tribes capabilities of Respcnding to Radiological Transportation Incidents On November 16, 1988, SGTB/NMSS met with indiana University (contractor) to review the status of the survey of the 50 states and 15 selected Indian Tribes beinD conducted to gather information with regard to their preparedness to respond to radiological transportation incidents. The project director reported that responses to a questionnaire designed to determine overall capability, e.g., training, equipment, procedures, etc., is issantially complete. The next step is to have participants in the survey verify the information provided.

It is expected that a final report will be completed by July 1989.

HIGH-LEVEL WASTE MANAGEMENT NRC/ DOE Meeting on DOE Quality Assurance Program Description (OAPD)

On November 18, 1988 members of the Division of High-Level Waste Management (HLWM) staff met with representatives from the Department of Energy (DOE), and the State of Nevada.

The purpose of the meeting was to discuss the HLWM staff coments on the DOE Quality Assurance Program Description (OAPD) and proposed changes that DOE intends to make on the "Nevada Nuclear Waste Storage Investigations Quality Assurance Plan" (NNWSI/88-9). The HLWM staff coments on the QAPD were provided in a conference call with DOE on November 4, 1988.

The proposed changes to NNWSI/88-9, which have been reviewed and accepted by tii.-

HLWM staff, were to: (1) address six open items in the HLWH safety evaluation; and (2) make changes where DOE believed NNWS! was too restrictive. A summary of the meeting will be issued shortly.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending November 25, 1988 PREVENTING DAMAGE TO REACTOR CORES Integrated Surveillance and Control System Workshop During November 15-16, 1988, Leo Beltracchi chaired the Integrated Surveil-lance and Control System Workshop (ISACS) in Halden Norway. The Halden Project, in which the NRC is a participating member, presents ISACS as a practical demonstration of a fully computerized control room. A prototype version of ISACS will be in operation at Halden during the early 1990's.

The purpose of the ISACS workshop was to conduct a critical design review of work to date and to provide guidance for future work. The workshop focused on three espects of the design. The first aspect is the design and specification of the man-machine interface. The structure and display of large quantities of information received special emphasis from workshop participants, which included power plant operations. A second aspect is the concept of an "Intel-ligent Coordinator." The Intelligent Coordinator serves as a sof tware super-visor of ISACS activities consisting of a process data monitor, manager of operator support systems, and a manager of the operator interface.

Finally, the third aspect is the selection of hardware and software tools for ISACS, A sumary of the workshop's recomendations will appear in a Halden Report to be published in the near future.

On November 23, the AE00 Director, DOA Director, and DEIIB Chief, accompanied l

by the Deputy Executive Director for Regional Operations and the NRR Director, made a presentation to the Commission on the Diagnostic Evaluation Program (DAP) and its effectiveness. The presentation provided an overview of the Diagnostic Evaluation program, process, and results to date.

On November 21, DEIIB staff began preliminary planning and preparations for a lessons-learned meeting on the diagnostic evaluation process. The meeting, which has been scheduled for December 9, is intended to develop proposed changes to the DEP process to further enhance its efficiency and effectiveness, i

Technical Training The initial written Examination Techniques enurse for operator license examiners was presented in Richland, Washington on November 14-18, by a contractor from Battelle. This is the first of three new courser which deal with the techniques associated with the different components of licensing examinations. The TTC Program Manager was the techni:a1 monitor for the course which was attended by contracted operator license examiners. The course was considered successful, although some improvements will be considered for its next presentation on January 9-13, 1989. The second of the new courses, the Simulator Examination Techniques Course for operator license examiners, is scheduled for presentation 1

at the TTC during the week of November 28.

2 The TTC hosted a workshop to review and revise the reactor health physics training curriculum on November 17-18. The workshop was attended by managers and/or supervisors from NRR, Region I-V and the TTC. This workshop achieved consensus on a revised list of Inspection Manual Chapter 1245 training require-l ments for reactor health physics inspectors, tonical outlines for the necessAry new courses and existing courses requiring revision, and a prioritized list J

for development of new and revision of existing courses. The current and future approaches to Site Access Training and Site Access Refresher Training j

were also discussed.

ITEMS OF INTEREST OFFICE OF GOVERNMENTAL AND PUBLIC AFFAIRS WEEK ENDING NOVEMBER 25, 1988 INTERNATIONAL PROGRAMS:

Foreign Visitors On Monday Robert de Mazancourt, French Service Central de Surete des Installations Nucleaires, met with representatives from AE00 and IP to discuss the NRC response (NRR, AEOD and PA) to the French Questionnaire on the methods used in keeping the public infenned about safety of nuclear installations.

Meeting with UK Officials Deputy Director, IP met with Dr. David Lumley, First Secretary (EnergyHarold D On November 22, 1988 and Mr. Jonathan Temple of the British Embassy. The discussions were wide ranging on all aspects of US-UK safety cooperation. Principal areas of interest to Dr. Lumley are current NRC activities in security, safety and safeguards.

Interest was expressed in having a UK assignee work at NRC and in conducting a fonnal US-UK technical exchange meeting sometime in 1989. Dr. Lumley will follow up with GPA on these initiatives in the coming months.

Texas Emergency Planning Meeting Charles Hackney, Region IV State Liaison Officer, attended a r.eeting with NRC and FEMA Region VI officials, and representatives from the State of Taxas, the South Texas Project, and the Comanche Peak Steam Electric Station in Austin, Texas on November 22, 1988. The purpose of the meeting was discuss offsite emergency planning exercise objectives and to plan a joint NRC/ FEMA emergency planning workshop tentatively scheduled for early June 1989.

A Notice of Violation and Proposed Imposition of Civil Penalty and Order to Show Cause Why License Should Not be Modified in the amount of $1.950 was issued November 18, 1988 to E. L. Conwell and Company, Bridgeport, Pennsylvania.

i The action was based on violations including)(1) failure to secure or maintain constant surveillance of a nuclear gauge, (2 failure to secure the source lock on a gauge when not in use (3) transport of a gauge without the gauge being accompanied by adequate shipping papers, ed transmittal of inaccurate information to the NRC.

(EN88-99) l An Order Modifying License and a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $80,000 was issued November 21, 1988 to Toledo Edison Company (Davis-Besse Nuclear Power Station). The Notice of Violation was based on the actions of a fomer supervisor who deliberately laid off a QC inspector for raising a safety issue. The Order Modifying iicense directs the licensee to notify the NRC should the former supervisor become involved in safety-related activities at the Davis-Besse Station.

Enforcement Conference with Washita Valley Enterprises. Inc.

The NRC Pegion IV staff conducted a telephonic enforcement conference on November 22, 1988 Washita Valley Enterprises. Inc. The purpose of the enforcement conference was to discuss the 10 apparent violations identified during NkC's October 13, 1988, inspection of the Itcensee's Oklahoma City facility. The licensee utilizes sealed cesium-137 sources in tube wall calipers for the measurement of pipe wall thickness. NRC's special, unannounced inspection resulied from a Region IV materials program initiative involving telephonic inquiries of infrequently inspected gauge licensees. During the telephonic inquiry, NRC learned of the eneuthorized replacement of the radiation protection of ficeri consequently, a Region IV materials inspector was dispatched to asskss the licensee's licensed activities.

Waterford 3 following)a 26-day outage Waterford 3 went critical on November 16, 1988, to resolve the noise problem in the B Steam Generator (SG and repair leaking reactor coolant pump (RCP) seals. On November 19, 1988, the unit achieved 100 percent power. No detectable noise is emanating from the B SG and all RCP seals are functioning properly.
