CP-87-29 (FINAL REPORT)

Gentlemen:

On December 11, 1987, we notified you via our interim report logged TXX-7081 of a reportable item involving instrumentation and controls used for isolation l

of component cooling water (CCW) to the reactor coolant pump (RCP) thermal i

barrier heat exchanger.

This is a final report submitted to provide additional information describing the cause of the deficiency and corrective actions of a reportable item under the provisions of 10CFR50.55(e).

The cause of the deficiency appears to have been a design error on the part of the original design organization.

Due to the unique operational requirements and alping arrangement of CCW to the RCPs, we consider this deficiency limited to t1is installation.

Design Change Authorizations (DCAs) 56897 and 60984 have been initiated to implement the corrective action specified in our last report.

The criteria for detection and isolation of the rupture of a thermal barrier is provided in Design Basis Document, DBD-ME-229, entitled."Component Cooling Water System."

In addition, DBD-ME-028, "Classification of Structures, Systems, and Components," has been issued to provide criteria related to this issue.

In order to prevent the recurrence of this condition, further plant modifications shall be evaluated using this DBD criteria to ensure conformance to CPSES design commitments.
