FASTENER TESTING TO DETERMINE CONFORMANCE WITH APPLICABLE MATERIAL SPECIFICATIONS

Dear Mr. Martin:

This letter is submitted in response to the requirements of NRC Compliance Bulletin No. 87-02.

The data obtained for the 55 fasteners tested are attached.

A synopsis of the findings, as well as a description of the Materials control program, follows.

All fasteners currently purchased for Rancho Seco are purchased quality Class I, procurement levels I, II or IV, and are identified by the specific standard (ASME SA-193, Grade B-16, ASTM A354, Grade BD, etc.) on the Purchase Order (PO).

Procurement levels I and II are made from authorized suppliers and includes either a source inspection or a l

Certificate of Conformance, as well as a receipt inspection.

l Procurement level IV is the "commercial grade" purchase, but includes a receipt inspection to insure the fasteners l

received are identified as required by the applicable codes and standards.

Procurement of fasteners at level IV is limited to 1/4 inch and smaller.

"Commercial grade" fasteners were previously purchased non-quality with no l

receipt inspection.

l Arrival on site is documented by the Quality Control (QC) organization through the receiving inspection data report l

(RIDR).

This inspection verifies the received materials meet l

all of the PO requirements.

This review includes, as I

applicable, certified material test reports, NDE certifications, and certificates of conformance.

All heat numbers are recorded in the Material Traceability Cross l

Reference log maintained by QC, and includes both the RIDR I

and PO numbers for cross reference.

acceptance tags each fastener package for tracebility into the field, and completes the RIDR.

The material is then returned to the receiving warehouse personnel for delivery to the main warehouse and placement into the proper stock code location.

Issue to the field for installation is controlled by the Stores Issue / Reservation Request (SI/RR).

The SI/RR provides a cross reference between work request (WR) number and stock code number.

All fasteners previously purchased to non-quality Class I requirements are requalified and subject to the dedication of commercial grade items process prior to issue to a quality Class I WR.

The process an described above ensures only approved fasteners are installed in quality Class I systems here at Rancho Seco.

Selection of fasteners to satisfy 87-02 requirements was performed by a team consisting of three SMUD representitives and one NRC Inspector.

This sample included both quality Class I and non-Class I fasteners with a variety of material grades.

The fasteners were tagged for identification and sent to Anamet Laboratories, Inc. of Hayward, California, a firm included on the District's Authorized Suppliers List.

Each fastener was tested under both the requirements of 87-02 and the ASTM /ASME requirements applicable to the stock code number from which the fastener was selected.

The test results, included in Attachments 1 and 2, identified one quality Class I and three non-Class I stock code numbers where the fastener did not meet the chemical or physical test requirements associated with material purchased for that stock code.

A description of each failure is presented below.

Sample number RS-012-Q, obtained from stock code number 038315 must meet ASME SA-193, grade B16 requirements.

The bolt passed the chemical, tensile strength, yield strength, and reduction of area test requirements, but failed to meet the 18% minimum elongation requirement, failing at 16%.

This failure has been evaluated and determined to be acceptable since all design conditions at Rancho Seco lie in the elastic range for bolts, not the plastic range where the failure occurred.

GCA 88-030 The second sample failure occurred in stock code number 026736, sample RS-001-NQ, a bolt originally purchased non-quality commercial grade.

The stock code requirements call for ASME SA193, grade B7, while the bolt head markings indicate the bolt was manufactured to SAE J429, grade 5 requirements.

This bolt failed the chromium and molybdenum requirements for ASME SA193, grade B7, but are acceptable under SAE J429, grade 5 standards.

The third failure occurred in stock code number 02481, samples RS-029A-NQ, and RS-029B-NQ, required by the stock code to meet ASTM A194, grade SM hex nuts standards.

These nuts were purchased non-quality.

The samples actually were ASTM A194, grade 8 hex nuts.

RS-029B-NQ failed the nickel requirement for grade 8, 11.44% actual to 10.65% required.

l This is considered acceptable since it adds to corrosion resistance.

The final failures occurred in stock code 024826, samples RS-027B-NQ, RS-027D-NQ, stock code required ASTM A194, grade 8M hex nuts.

The nuts were purchased non-quality.

The nuts are acutally ASTM A194, grade 8.

Sample RS-027B-NQ failed the e

carbon requirements for grade 8 with an acutal 0.11%

concentration versus 0.09% required.

This is considered acceptable for grade 8 since the nuts are not used in welding applications.

In summary, the single failure of a quality Class I fastener was evaluated by Nuclear Engineering and determined to be acceptable.

The failure of non-Class I fasteners is the result of an inappropriate grade of fastener being in the stock code bin.

The fasteners were evaluated based on the material requirements associated with the actual markings on the fastener.

These evaluations determined acceptability i

based either on the requirements imposed by the actual markings, or through an engineering evaluation of the failed 3

j requirements.

I All fasteners from stock codes that contained failed l

components have been removed from the warehouse shelves and placed in "quarantine" awaiting disposition.

Material control personnel will review the complete purchase and issue history for each stock code that produced a failure to i

determine why the improper grade materials were found in the bins.

Based on this evaluation, a determination of 4

additional sampling or other corrective actions necessary to eliminate this problem will be made.

A program to remove all l

non-Class I fasteners is in progress and will be completed by June 30, 1988.

It is believed this program will correct this improper materials problem.
