responses to the items contained in the Notice of Violation are hereby submitted:
Water Chemistry Instrument Quality Control Charts Technical Specification AC 7.4 states, "Written procedures shall be established, implemented, and maintained covering the activities referenced below:

, June 22, 1988 The licensee's Water Chemistry Procedure-356, "Analytical Control Charts," Issue 1,. da td Octobcr 21, 1987, paragraph 5.8.3 requires that, when ar, out-of-control situation occurs, analysis must be stopped until the problem has been identified and:
resolved, after which the frequency should be increased for the-next few quality control checks and the problem and. solutions must-be documented.
Paragraphs 5.8.4 and 5.8.5 further define corrective actions to be taken following identifiestion of out-of-control situations which include the making of new instrument control charts utilizing new standards, duplicates, and spikes and all analyses since the last in-control point must be evaluated and appropriate action taken.
Contrary to the above, the NRC inspectors determined on April 13, 1988, that the water chemistry instrument quality control charts for analyses for chloride by ion chromatography, silica by spectroscopy, and iron and copper by graphite furnace atomic absorption had not been properly evaluated and several examples of out-of-control situations existed for each of the above analyses during the time period November 11, 1987, through April 13, 1988, with no documented evaluation or corrective action taken as directed by procedure.
This is a
violation.

(1) The Reason For The Violation If Admitted:
Chemistry Unit personnel failed to follow the procedure as required.
(2) The Corrective Steps Which Have Been Taken And The Results Achieved:
All Chemistry personnel have been specifically instructed in the importance of following procedures and in the utilization of the procedure revision system to correct procedure problems prior to continuing.
It was also emphasized that it is mandatory to follow procedures and that failure to do so will result in appropriate disciplinary action.
Control chart data has been evaluated and new control charts developed as required.
The control chart procedure (WCP-356) was revised to clarify several procedural-functions which had caused discrepancies in interpretation within the Chemistry Unit.
Also, computer data trending software has been modified which has made control chart acceptance criteria readily available to personnel for reference during performance of laboratory testing.

The corrective actions that have been taken will serve to avoid further violations.
