Response to Combined Inspection Report Nos. 50-277/87-22; 50-278/87-22 and 50-277/87-25; 50-278/87-25 Concerning Shutdown Cooling Isolations
Dear Mr. Kane:
This letter is in response to Combined Inspection Report Nos. 50-277/87-22; 50-278/87-22 (dated October 2, 1987) for the inspection period from July 18, 1987 to August 28, 1987 and Combined Inspection Report Nos. 50-277/87-25; 50-278/87-25 (dated November 5, 1987) for the inspection period from August 29, 1987 to October 16, 1987.
These inspection reports identified a concern with respect to the number of shutdown cooling isolations that have occurred during the above identified inspection periods and requested that we analyze the root causes and provide corrective actions to reduce the frequency of occurrence.
The Philadelphia Electric Company staff recently completed a review of these events.
The root causes for the eleven events determined by this review are identified below:
a) 6 events were the result of personnel error, b) 1 event was the result of an act of nature.
c) 1 event was the result of an equipment failure, d) 1 event was the result of a lack of control of work outside the station.

A root cause could not be determined for the remaining event.
Several corrective actions, in addition to those mentioned in the Licensee Event Reports, have been identified for consideration.
Following management's review, it has been concluded that a formal root cause evaluation is warranted to confirm the completeness of the review.
In order to expedite the completion of this analysis, an outside expert consultant has been employed to work with our staff.
Completion of this formal root cause analysis followed by a submittal to the NRC is expected by February 26, 1988.
