COMANCHE PEAK STEAM ELECTRIC STATION DOCKET N05. 50-445 AND 50-446 TERMINATION NOT PER DRAWING.

SDAR:

CP-87-123 (FINAL REPORT)

Gentlemen:

On November 25, 1987, we verbally notified your Mr. R. F. Warnick of a possible deficiency involving wire terminations not in accordance with design drawings. Our most recent interim report was logged TXX-7128, dated 1

December 28, 1987. We are conservatively reporting this issue under the provisions of 10CFR50.55(e).

The required information follows.

l DESCRIPTION During third party inspections and observations by project personnel, cables have been identified in safety related applications which are not terminated in accordance with the design drawings.

As discussed in the Corrective Action Request issued to address these concerns (CAR 87-049), these conditions are

.the result of. changes issued by drawing revisions versus Design Change Authorizations (DCAs). Changes not issued by individual DCAs are not readily identifiable for implementation by construction.

This issue is generic to all changes issued by the drawing revision process without prior issuance of a DCA.

SAFETY IMPLICATION l

Failure of cables to perform as intended by design could result in an inability of associated safety-related systems to perform as recuired for safe l

operations and shutdown.

Had the condition remained uncorrectec, it could l

have adversely affected the safety of plant operations.

TXX-88049 January 11, 1988 Page 2 of 2 CORRECTIVE ACTION In order to correct this deficiency the specific cables identified will be reterminated no later than July 5,1988.

In addition, external field wiring terminations'will be validated during the Post Construction Hardware

. Validation-Program.

The Prestart Test Program will validate that safety related systems function in accordance with the approved design.

Deficiencies between previously implemented design documents and the current installation will be documented in accordance with site corrective action procedures.

To prevent-recurrence, TV Electric Design Change Authorization Procedure ECE 5.01, Rev. 0 was issued effective March 2, 1987.

This procedure established the mechanism for documenting and controlling design changes.

Subsequent to the implementation date of this procedure, changes cannot be implemented by a drawing revision.
