SUBJECT: NRC INSPECTION REPORT 50-133/98-04
Dear Mr. Rueger:
This refers to the inspection conducted over three weeks during the period September 1,1998
to October 22,1998 at your Humboldt Bay Power Plant No. 3 facility. The enclosed report
presents the scope and results of that inspection.
The focus of the inspection was the activities associated with the removal of the 250' ventilation
stack. This included reviewing the safety evaluation and work procedures prior to the start of
work and observance of work activities in progress. In eddition, areas related to occupational
radiation exposures, radwaste shipping, self assessments and maintenance programs were
reviewed. No violations of NRC regulations were identified.
In preparation for the stack removal project, we noted that the effort of the nuclear quality
services organization was worthy of special recognition. The alert attention of the auditors
during their independent assessment of the program documentation resulted in the discovery
'
that the wrong crane rating load chart had been provided by the crane vendor. This chart was
to be used by the licensee staff to determine the allowable weight of the stack sections to be
removed. Had this error not been detected prior to the start of work, the margin of safety for
the crane activities would have been significantly reduced, increasing the potential for
'
overloading the crane.
