COMANCHE PEAK STEAM ELECTRIC STATION DOCKET NOS. 50-445 AND 50-446 SOLIO STATE SAFEGUARDS SYSTEMS (5555) INTERNAL WIRING TERMINATIONS SDAR: CP-87-49 (INTERIM REPORT)

On July 22, 1987, we verbally notified your Mr. 1. Barnes of a deficiency involving a condition cited at another nuclear facility in which an open electrical equipment.connection on onethis We are reporting crimp issue lug caused under a failure inofload the provisions sequencing (e).

10CFR50.55 The required infonnation follows.

DESCRIPTION OF PROBLEM Vitro Corporation letter (CVT 0793 dated June 16,1987) to the US NRC provided details on problems identified at South Carolina Electric and Gcs Company (SCE & G). The problems involved deficiencies in crimped lug electric connectors in load Sequencers, and was reported by SCE & G under 10CFR21.

Reviews conducted by Vitro concluded these deficiencies were due to inadequate shop and inspection controls in place at Vitro prior to April, 1979.

TU Electric was supplied by Vitro with four Load Sequencers which are presently installed in the Solid State Safeguard System (SSSS). Unit 1 Load Sequencers were delivered in December, 1977 and Unit 2 Load Sequencers in July,1979, coinciding with the period Vitro's controls were inadequate.

The SSSS Load Sequencers are currently under Operations' control. The internal wiring of the Load Sequencers was inspected by operations for proper workmanship based on inspection criteria recommended in Vitro's Manuf acturing Process Standards MPS 136 and CPSES Electrical Specification (ES) No. 100.

Test Deficiency Report (TDR) N0. 5722 was issued identifying deficient wire crimp lugs.

SAFETY IMPLICATION Failure of the SSSS Load Sequencers to perform as designed could result in failure of safety related systems to perform as required for safe plant <

operations and safe shutdown.

TXX-88021 January 11, 1988 Page 2 of 2 CORRECTIVE ACTION Startup Work Packages (SWP) No. 4094 and 4095 have been initiated 'to reterminate the deficiencies.

Our next report will provide a schedule for completion of these activities in Unit-1 and Unit 2 and will be submitted no later than March 29, 1988.
