COMANCHE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET NOS. 50-445 AND 50-446 CLASS 1E SEPARATION VIOLATIONS SDAR: CP-87-13 (INTERIM REPORT)

Gentlement On May 15, 1987, we verbally notified your Mr. 1. Barnes of a deficiency involving Class 1E separation violations resulting from the removal of or niodification of non-safety class cable tray covers after acceptance by CPSES Quality Control. We are conservatively reporting this issue under the provisionsof10CFR50.55(e).

The required information follows.

Description The deficiency, as documented by Non-conformance Report (NCR) E86-101601 and Corrective Action Report (CAR) 064, documents separation violations resulting from the removal of or modification of nor,-safety related cable tray covers removed and/or modified after final acceptance by CPSES Quality Control.

The cause of this deficiency is a lack of specific procedural requirements which monitor the reinoval of non-safety related cable tray covers used as separation tiarriers.

Safety Implications failure of cable tray and or cables to perform as intended by design could result in the inability of safety-related systems to perform as required for safe operations and shutdown.

~Affected cable tray covers will be replaced. As noted below, additional separation barrier violations will be documented using NCRs and corrective actions implemented in accordance with the disposition of the required activities.

To prevent recurrence, inspection Removal Notice (IRN) Procedure, CP-CPM-6.10, paragraph 1.2.3, has been revised to require the issuance of IRN's for the removal of all tray covers, irrespective of safety class designation.

The training of ap)licable construction _ personnel in accordance with the revised procedure has

Supporting information is available at the CPSES site for your inspector's review. We expect to include a schedule for implementation of corrective actions in our next report which will be submitteu no later than April 8, 1988.
