Dear Mr. Davis:

This letter is in response to the inspection conducted by Messrs.

M. Jordan and R. Kopriva on September 22 through November 3, 1987, of certain activities at LaSalle County Station. Reference (a) indicated that certain activites appeared to be in noncompliance with NRC requirements.

The Commonwealth Edison Company response to the Notice of Violation is provided in the Attachment.

.- ATTACHMENT Technical Specification 6.2.A.1 requires that written procedures be prepared, approved and adhered to, including Administrative Procedures.

LaSalle Administrative Procedure LAP-100-30, "Independent .

Verification," Step F.3.e.1 states, "When lifting leads, always use the wiring diagrams and wire color indication if it is available." LAP-240-6, "Temporary System Changes," paragraph E.5 states, "All Temporary System Changes will be installed using the wiring diagrams and all terminal wires will be verified to be correct according to the color code of the wires per the wiring diagram and physical installation at the terminal board."

Contrary to the above, on October 17, 1987, electrical maintenance personnel failed to adhere to LAP-100-30 and LAP-240-6 and used electrical schematics instead of electrical wiring diagrams to remove a failed relay in the Unit 2 Residual Heat nemoval (RHR) system. A relay in the Reactor Water Cleanup System (RWCU), which was in series with the RHR relay was then actuated on loss of power when leads were lifted to the RHR relay, causing an RWCU isolation.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED Upon receipt of the isolation, the work in progress was stopped and the appropriate wiring diagrams were reviewed. All leads were relanded, the isolation signal was reset, and the Reactor Water Cleanup System was restarted.

. CORRECTIVE ACTION TAKEN TO AVOID,F_URTHER V,IOLA. TION A discussion was held between the department head (Master Electrician) and the electrical maintenance foreman involved in this event.

The discussion emphasized that the inattention to detail identified in this event was unacceptable. In addition, the expectations for a supervisor were reviewed with the foreman, including the need to strictly adhere to procedures. The electrical maintenance foreman recognized that his performance in this event was unacceptable.

Electrical maintenance personnel were trained on this event with emphasis placed on attention to detail. In addition, the training emphasized the strict adherence to procedures and the use of electrical wiring diagrams when working in electrical circuits in accordance with LAP-100-30 (Independent Verification) and LAP-240-6 (Temporary System Changes).

Details of this event have been provided to the instrument maintenance, technical staff, and operating departments for dissemination of lessons learned.
