This letter is in response to the Notice of Violation received as a result of the inspection conducted by Messrs. R. E.

- Michaud, R. P. Mullikin and K. L. Heitner during the period March 1-31, 1988. The following responses to the items contained in the Notice of Violation are hereby submitted:
1.
Emergency Diesel Generator Valve Lineup Licensee Procedure IVL-92-04 Issue 1, "Integrated System Valve Lineup and Checklist for Emergency Diesel Generator" lists the correct valve lineup for the emergency diesel generators.
l Contrary to the above, on March 21, 1988, the NRC inspectors found two valves in the emergency diesel generators air starting system to be in a position which was not in accordance with the l
applicable procedure or drawing.
This is a Severity Level IV violation.

The violation is admitted.
A thorough investigation was conducted as to why the two valves were left open.
No cause could be determined.

A complete diesel generator system valve lineup, with independent verification, was completed.

4 mispositioned valves were identified. As a result of this event, the four cross-connect isolation' valves and two additional. valves in the diesel generator air start system have been' sealed shut and placed on the plant sealed and critical valve checklist.
(3) The Corrective Steps Which Will Be Taken To Avoid Further violations:
-The sealing of-the valves noted above will preclude unintentional changes in the air start system valve lineup.

- addition, system valve lineups, with independent verification, will continue to be performed on a random system basis as directed by the Operations Manager. The intention of random system valve lineups is to increase i
operator awareness and provide an ongoing check of system valve configuration.
(4) The D6te When Full Compliance Will Be Achieved:
Full _ compliance was. achieved on March 31, 1988 when the noted valves were sealed and placed on the plant sealed and critical valve checklist.

be performed on an ongoing basis.

Deficient Procedure System Operating Procedure 45-01, Issue 21, "Fire Protection 4
System," and Integrated Valve Lineup 45-01, Issue 1, "Integrated 2
L System Valve Lineup and Checklist for S0P 45-01," specify the valve lineups for the engine driven firewater pump for normal l
operation.
Contrary to the above, three valves (unnumbered) in the cooling water line to the engine driven firewater pump are not identified on the plant valve lineup check lists.

Valve positions were verified as correct.
The valves were temporarily tagged to prevent manipulation from the verified positions.
General Service Action Request (GSAR) number 2154 was initiated to provide for permanent numbering of the noted valves.
(3) The Corrective Steps Which Will Be Taken To Avoid Further Violations:
Change Notice (CN) number 2814 will be implemented in response to GSAR-2154. The CN, which is being prepared by the Nuclear Engineering Division, will assign plant component numbers to the valves and provide for the installation of permanent tags.
Administrative controls present in the design change process will preclude this type of oversight in the future and trigger necessary procedural revisions to plant Standard Operating Procedures.
(4) The Date When Full Compliance Will Be Achieved:
CN-2814 will be implemented by August 15, 1988.

Mishandling of Control Rods Licensee Nonconformance Report 86-218 in Item 1 of Guidelines states, "Avoid touching the control rod with uncovered hands to minimi.ie the amount of foreign materials deposited on the surface."
Contrary to the above, on March 23, 1988, licensee personnel working in accordance with NCR 86-218 handled control rods with bare hands.

The Reason For The Violation If Admitted:
The violation is admitted. The control rods were mishandled due to a combination of errors. Workmen involved in the job were inattentive to detail. Workmen also failed to review the work plan in sufficient depth. Although the work plan included written guidelines which specified that the control rods not be touched with uncovered hands, the guideline was not highlighted as a caution.
(2) The Corrective Steps Which Have Been Taken And The Results Achieved:
Non Conformance Report (NCR)88-063 was initiated to address the possible introduction of foreign materials onto the surface of the control rods being worked.
NCR 88-063 provided for cleaning of the mishandled control rods with an appropriate cleaning solution.
g-O P-88217 June 17, 1988 A meeting was held between the work crew involved in the incident and plant management represented by the Superintendent of Technical Services Engineering.
Public Service Company's comitment to follow procedures, including work plan guidelines, was emphasized.
The practical importance and control of. stress corrosion at Fort St. Vrain was discussed in detail. Workmen were informed that. future 1
incidents of this kind would result in the initiation of progressive discipline.
No further work was performed on the control elements with uncovered hands.
(3) The Corrective Steps Which Will Be Taken To Avoid Further Violations:
Supervisory personnel for the Fuel Deck work crews have been instructed to maintain a high level of management attention to work crew performance.
(4) The Date When Full Compliance Will Be Achieved:
Full compliance was achieved with the closure of NCR 88-063.
This NCR provided for cleaning of the control elements. NCR 88-063 was completed on April 13, 1988.
