NRC ROUTINE RADIATION PROTECTION INSPECTION REPORTS
50-456/98015(DRS); 50-457/98015(DRS) AND NOTICE OF VIOLATION

On October 22,1998, the NRC completed an inspection at your Braidwood Generating Station,
Units 1 and 2. The enclosed report presents the results of that inspection.
The !nspection was an examination of activities conducted under your license .3 c,ey rebte to
radiation safety and to compliance with the Commission's rules and regulatione., s.nd with the
conditions of your license. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with personnel, and
observation of activities in progress.: Specifically, the inspection focused on the radiological
controls for Unit 1 outage activities and the steam generator replacement project, and included a

. review of the radiological environmental monitoring program.

The inspection determined that the overall radiological controls and the As-Low-As-Is-
Reasonably-Achievable (ALARA) program were effectively imple nented during the outage and

the steam generator replacement project, and dose was maintair.Mi reasonably low. The
radiological environmental monitoring program was also well impletanted, with no discernable
d
impact on the environment from plant operations. However, an Noleted problem was identified
with the control of radiographic operations and problems with worker practices were noted.
Also, poor work group communications and lack of recognition of simultaneous evolutions led to
fuel handling building cross town area contaminations.
Based on the results of the inspection, two violations of NRC requirements were identified, as
specified in the enclosed Notice of Vioiction (Notice). The first violation involved the failure to
ensure that all personnel had been instructed to leave an area rii 10 beginning radiography.

- Two workers were not _ notified to leave an area prior to the beginning of radiography. As a
result, they were insid 3 a posted radiological boundary during the performance of radiography
and could have received unnecessary radiation exposures. The second violation involved the
failure of workers to properly follow the dress requirements of the radiation work permit that
govemed the work activity. Specifically, workers in containment failed to wear the proper gloves
as specified in the work permit, which could have resulted in contamination of the workers.

The NRC has concluded that information regarding the reason for the violations, the corrective
actions taken and planned to correct the violations and prevent recurrence, is already
adequately addressed in the subject inspection report. Therefore, you are not required to
respond to these violations unless the description of the problems as described in the inspection
report does not accurately reflect your corrective actions or your position. In that case, or if you
choose to provide additional Information, you should follow the instructions specified in the
enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC'S " Rules of Practice," a copy of this letter, the
enclosures, and your response to this letter will be placed in the NRC Public Document Room
(PDR).
