ENCLOSURE 1 NOTICE OF VIOLATION Westinghouse Electric Corporation Docket No. 70-1151 Columbia Nuciear Fuel Plant License No. SNM-1107 i During the Nuclear Regulatory Comission (NRC) inspection conducted on September P.6-30, 1988, a violation of NRC requirements was identified. In -

accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1988), the violation is listed below:  ;

License No. SNM-1107, Condition 9 requires that licensed materials be used in accordance with statements, representations, and conditions contained in Chapters 2, 3, and 4 of the Application dated January 4,1985, and  !

supplements thereto.

Paragraph 2.5 of the Application states that implementing procedures for i the Radiological Contingency Plan shall be maintained to implement the  :

Plan.

I Section 7 of the Emergency Plan Revision 1, dated December 23, 1987, states that "certain copies of the implementing Procedures will be issued  ;

to plant personnel on a controlled distribution list. Updated copies of i all revised procedures will be provided to all controlled copy holders."  :

Contrary to the above requirement, during the week of September 26-30 1988, six controlled copies of the Site Emergency Plan and implementing Procedures were incomplete or had not been maintained in an up-to-date '

manner. Two copies in the Control Room contained old revisions in the tabbed locations while the most recent revisions were placed in the back of .

1 the book unlabeled. One copy of the Plan at the Main Guard Station had l old revisions and the latest revision comingled. One Plan located in the  ;

Emergency Brigade Trunk did not contain the latest revision issued on [

-) August 12, 1988. One Plan copy located in the Health Physics Emergency i Cabinet was incomplete and had Procedure CSEP 12 A titled "Contaminated (

Casualities" missing. One copy assigned to an individual was missing Appendix B titled "Calculations of Dose Comitment". In addition, several i procedures in contrclied copies were found to have double sided procedures copied only on one side.

This is a Severity level IV violation (Supplement V!ll).
