ENCLOSURE RESPONSE TO NRC INSPECTION REPORT NOS. 50-327/83-44 AND 50-328/88-44 F. R. McC0Y'S LETTER TO S. A. WHITE DATED OCTOBER 28, 1988 Violation 50-327, 328/88-44-02 "Ter.hnical Specification 6.8.1 requires that written procedures be establishef, implemented and maintained for those activities associated with

equipmer, listed in Appendix 'A' of Regulatory Guide 1.'3, Povision 2, which requires that maintenance that can affect the performance of safety-related equipment be properly preplanned and performed in accordance with written precedures.

Work request, WR 8206449, contained a caution statement to be adhered to while performing a position calibration on the Unit 2 CIPA' incore detector.

The caution specified that the storage reel be locked wnen using the drive unit to drive the detector to the storage location.

Contrary to the above, on September 9. 1988, licensee personnel failed to comply with the precautions listed in WRB296449, resulting in an inadvertent withdrawal of the incore probe from its tube enclosure.

This violation occurred as a result of personnel error.

4 The individuals involved failed to secure the cable storage reel for the "A" incore detector with the locking pin before loosening the spring-loaded holddown reel.

This personnel error, which could have been avolded by adherence to the p ecaution referenced in the above notice of violation, occurred in par t as a result of several factors.

First, the work request did not provide detailed work i

i inst *uctions.

Instead, the Individuals themselves were relled upon to perform i

this maintenance activity based on their knowledge of the detailed 1

instructions contained in a two-volume vendor manual that was not avaliable at the job site.

i Additionally, the lack of a prejob briefing, the addition of a full face mask respirator to the dressout requirements (which seriously affected communications), and the absence of sufficient lighting in the work j

area all contributed to this event.

As a result of this event, SQN has implemented Halntenance Instruction 1.'. 3.8, f

"Incore Flux Detectors Removal and Installation." This procedure, alon) with providing detalled work instructions, provides 111ustrations, warnings, and dou'ule person verifications that the locking pin is installed when required.
