1 ENCLOSURE REACH BOTTOM ATOMIC POWER STATION STATUS REPORT FOR 1HE PERIOD October 19 - November 7, 1988 1.0 P_lant Status As of 8:00 a.m. on November 7, 1988, Units 2 and 3 are in a cold condition per the March 31, 1987, NRC Order with coolant temperatures less than 212 degrees Fahrenheit. The Unit 3 core is completely off loaded into the spent fuel pool.

2.0 Facility Operations Summary Unit 2 is in day 605 of its seventh refueling outage that began March 13, 1987. Unit 2 maintenance repair work is in progress on plant systems required for restart. Sixteen of fifty Unit 2 system windows are closed and turned over to operations. The Unit 2 reactor pressure vessel hydrostitic test is the next critical path item. Unit 3 is in day 404 of its seven+h refueling outage that began October 1, 1987. Unit 3 m11ntenance and modification work is underway on 27 of 41 plant systems.

Four plant systeos have been closed.

3.0 Items of Special Interest 3.1 On October 19, 1988, the Peach Bottom Atomic Power Station Safety Evaluation Report was issued. In this report, the NRC concluded that the Philadelphia Electric Company "Plan for Restart of Peach Bottom Atomic Power Station" is acceptable and meets the requirements of the March 31, 1987 Shutdown Order.

3.2 On October 19, 1988, the NRC issued the Systematic Assessment of Licensee Performar.ce (SALP) Board Report Number 50-277/87-99; 50-278/87-99. The SALP report reviewed and evaluated Peach Bottom performance during the period June 1,1987 through July 31, 1988.

The SALP management meeting was held at Peach Bottom on November 1, 1988. At this mee;ing, the NRC presented the SALP evaluation and the licensee responded to the noted weaknesses in each of the functional areas. The licensee will respond with written comments to the SALP report.

3.3 On October 21, 1988, Philadelphia Electric Company announced that there would be a deley in the readiness for restart of Units 2 and 3 to allow further upgrade in areas such as operator training, managerial and supervisory effectiveness and plant security. These actions will delay restart readiness of Unit 2 untti the second quarter of 1989 and of Unit 3 until the third quarter of 1989.

.,..,., i 3.4 On November 1, 1988 Philadelphia Electric Company briefed the NRC Peach Bottom Restart Assessment Panel on the results of the September INPO inspection. INPO comments included a notable reduction in con-taminated areas and radiation exposure, and an improvement in teamwork among plant workers. INPO also reported that operators need additional i simuls'or training to properly respond to certain plant events, that manageiont and shift supervision need:, to take more effective action to corr et operational and administrative problems, that procedures need to ae upgraded to better aid operators in determining plant status, and that improvements are needed in assessing and communicating  !

performance standards. These items will be the subject of future NRC inspection reports.  ;

4.0 Other Items of Interest  !

4.1 The licensee completed an additional phase of licensed operato; training on the Peach Bottom simulator in Columbia, Maryland, on ,

November 4, 1988. Each shift crew will receive an additional three ,

days of simulator training prior to crew evaluations that will be

conducted by INPO and NRC in December 1988.

4.2 On October 27, 1988, the licensee made a four hour telephone notification to the NRC regarding a Unit 2 engineered safeguard feature (ESF) actuation caused when power was lost to an electrical l panel. The power loss resulted from a personnel error during troubleshooting activities.

4.3 On October 29, 1988, the licensee made a four hour telephone ,

notification to the NRC regarding a Unit 2 ESF actuation caused when ,

power was lost to the reactor protection system (RPS). The power loss resulted when a contractor worker apparently bumped the RPS control panel.

4.4 On November 5, 1988, the licensee made a four hour telephone notification to the NRC regarding a Unit 2 ESF actuation that occurred on November 4, 1988. The actuation was caused by a personr.a1 error during survelliance testing.  ;

4.5 The licensee reported that both fire pumps were simultaneously  ;

inoperable on October 20, 1988. Written correspondence was '

subsequently provided in letters dated October 21, 1988 and November l 3, 1988. -

The above items will be the subject of future NRC inspection reports. }

5.0 NRC Activities During the period 5.1 Three NAC Resident Inspectors and a regional inspector were assigned  !

to monito. plant activities between October 19 and November 7, 1988.

, activities and demeanor, reportable events, surveillance testing, outage activities, maintenance, power ascension program and outstanding items.

5.2 Two specialist inspection reports were received during the period.

Inspection report (50-277/88-36; 50-278/88-36) concerned the annual emergency preparedness exercise and was issued on October 17, 1988.

The inspection was conducted September 26-28, 1988. No violations were identified. It was determined that the emergency response actions were adequate to provide protective measures for the health and safety of the public.

Inspection report (50-277/88-28; 50-278/88-28) concerned review of the licensee's operating procedures, non-licensed operator training, and on site and off site review committees. Also reviewed were selected correctivi action items of the licensee's Plan for Restart of Peach Bottom Atomic Power Stations Units 2 and 3, Sections I and II. The inspection was conducted August 29 - September 2, 1988. No violations were identified; however, it was noted that an auxiliary operator did not follow the explicit instructions of a routine test procedure, nor did he notify the chief operator of the deviation in the routine test.

5.3 One resident inspection report (50-277/88-34; 50-278/88-34) was receive 0 during the period. The inspection was conducted from Septembe* 3 to October 14, 1988. The report was issued on November 1, 1988. No violations were observed; however, the licensee was requested to respond to concerns regarding the surveillance testing program.
