SUBJECT: NRC INSPECTION REPORT 50-498/98-09; 50-499/98-09 AND NOTICE OF
VIOLATION

Dear Mr. Cottle:
On October 17,1998, the NRC completed an inspection at your South Texas Project Electric
,
Generating Station, Units 1 and 2, reactor facilities. The enclosed report presents the scope
and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the South
. Texas Project facilities was generally characterized by safety-conscious operations, sound
engineering and maintenance practices, and careful radiological work controls. However, one
. violation of an NRC requirement was identified. The violation was of concem because a work
=
instruction was changed to include inappropriate work instructions that caused a loss of
- feedwater flow to one steam generator and a resulting reactor trip.: The change to the work
instructions was inappropriately made as a minor change and received no review. Your review
of this event did not identify this significant contributing cause.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described i 1 detail in the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. Your response to this violation should include your
actions to address the inappropriate method used to change the affected work instructions.
The NRC will use your response, in part, to determine whether further enforcement action is
necessary to ensure compliance with regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room (PDR).
