Response to Notices of Violation and Notice of Deviation in Inspection Report 86-02 Gentlemen:

Appendices A and B of Inspection Report 86-02, Reference (b), identified one violation and one deviation, respectively. Below we have restated the findings and our responses in accordance with the instructions contained in the Notices.

Violation Criterion V, Appendix B,10CFR50, requires activities affecting quality to be prescribed by and accomplished in accordance with procedures of a type appropriate to the circumstances. The licensee's Quality Assurance Program,Section V.B.2, requires that applicable department, shall be responsible for the preparation, review, and implementation of all instructions and procedures affecting quality.

Contrary to the above, as of March 14, 1906, quality-affecting activities, relating to tne surveys of safety-related masonry walls that were conducted in response to IE Bulletin 80-11, were not performed utilizing controlled procedures and instructions. ,,

1. Corrective steos taken and the results achieved.

Maine Yankee does not consider a survey or plant walkdown, conducted ..

l to gather information on safety-related systems, components or I structures, to necessarily require a controlled instruction or procedure. Nevertheless, in this particular case, a procedure to i survey masonry walls which are located within the vicinity of (i.e.

distance equal to the wall height), or have attachments from safety-class systems, is appropriate. Accordingly, Procedure MYPP-1,

" Maine Yankee Masonry Hall (IEB80-11) Survey Procedure," has been developed for the walkdown of masonry walls. Findings from our recent walkdown of the plant (February, 1986) comply with the requirements of MYPP-1.

This procedure was issued on April 1, 1986 for future masonry wall surveys resulting from building alterations.

2. Corrective stens which will be taken to avoid further violations.

The results of the February, 1986 masonry wall survey and surveillance walkdowns have been incorporated into a controlled document, the Maine Yankee Masonry Hall Survey Manual. This-document includes Procedures MYPP-1 and MYPP-2 " Maine Yankee Masonry Hall Surveillance Plan". The latter procedure will be used to ensure that the condition of the subject walls has not changed since the previous inspection.

In accordance with a verbal commitment made during the inspection, Maine Yankee project personnel from Yankee Atomic Electric Company (YAEC), Nuclear Services Division, have completed retraining in the need and use of Engineering Instruction HE-106 " Procedures and Instructions".

3. The date when full como11ance will be achieved.

Full compliance was achieved on May 19, 1986 with completien of the training.

Murley, Regional Administrator MN-86-71 Deviation IEB 80-11, dated May 8, 1980, informed licensees that all masonry walls in each facility which are in the vicinity of or have attachments from safety-related systems should be identified and evaluated for structural adequacy.

In letters, dated June 30, 1980 and November 4, 1980, the licensee responded to the bulletin and indicated its commitment to complete actions delineated in the bulletin.

Contrary to the above, as of March 7,1986, five walls in existence at the time of issuance of the bulletin had not been identified by the licensee and -

evaluated as specified by the bulletin.

Maine Yankee Resoonse

1. Corrective steos taken and the results achieved.

The walls in question have been evaluated and added to the list of safety-class masonry walls at Maine Yankee. A draft copy of the revised list of safety-class masonry walls was provided to the inspector on March 7, 1986. Maine Yankee's letter, Reference (c),

dated March 25, 1986, submitted the updated list of masonry walls.

2. Corrective steos which will be taken to avoid further deviations.

A plant walkdown was performed to provide assurance that all-safety-class block walls had been identified. The results of the walkdown complied with the requirements of Procedure MYPP-1.

As an added measure to address the unresolved item (86-02-02) identified in Reference (b), a three-part program has been instituted to help ensure that future safety class equipment is not installed near masonry walls.

Harning notes have been added to the pertinent plant drawings.

Harning notes have been stenciled on the masonry walls.

YAEC-YNSD Hork Instructions HE-100, 101, and 103 have been revised to include warnings with regard to IEB 80-11 concerns.

Full conformance was achieved on March 25, 1986 with the submission of Reference (c).
