SUBJECT:

LOSS OF FEEDWATER EVENT STATUS - DAVIS-BESSE Attached for your information is our monthly status report of activities at Davis-Besse relating to the Loss of Feedwater Event on June 9, 1985.

Activities relating to corrective actions on equipment that malfunctioned on June 9,1985, and evaluations and repairs of safety-related hangers and supports are continuing. The licensee found that Raychem safety-related electric wire splices throughout the plant may be improperly installed.

Regarding the Reactor Coolant Pump (RCP) repairs, the licensee now plans to replace all four RCP shafts even though there are probably no cracks in the shafts. On June 2, 1986, at 12:01 a.m. (EDT), members of Local 245 of the International Brotherhood of Electrical Workers (IBEW) walked off the job after rejecting the licensee's latest contract offer.

Region III has been monitoring the strike activities. The unit outage completion date is now projected to be near the end of October 1986. The NRC Restart Test Review 4

Group has been on site reviewing the licensee's restart program and observing testing activities.

e REGION III STATUS REPORT DAVIS-BESSE LOSS OF FEEDWATER EVENT June 2, 1986 Plant Status The plant remains in cold shutdown.

The primary system is filled with a nitrogen bubble in the pressurizer.

Restart Activities MOVATS testing, environmental qualification work and other maintenance activities continue on the 167 safety-related motor-operated valves onsite.

The licensee has completed all planned work on 124 valves and returned them to service.

The NRC test review team continues to provide coverage of licensee testing activities. The licensee's System Review and Test Program (SRTP) includes 282 tests of which 26 will be performed following criticality. This program consists of 172 existing, or modified, surveillance tests and 110 new test procedures. Ninety-one tests have been performed to date. The licensee has decided to delay hot functional testing until after replacement of the Reactor Coolant Pump shafts.

Other Activities After inspecting the No. 2-1 reactor coolant pump (RCP) shaft, impeller-to-shaft capscrews, and impeller drive pins, the licensee has decided to replace the rotating elements (shafts and impellers) of the other three RCPs.

Destructive examination of the No. 2-1 shaf t did not reveal any cracking in the shaft.

Service Water Pump 1-1, one of three pumps in the system, is inoperable due to a sheared shaft and severe scoring of several shaft bearing surfaces. The Gould, two stage, centrifugal, deep-draft pump will be repaired when parts are received.

A licensee Quality Centrol inspector identified improper installation of Raychem electrical terminations on safety related equipment.

Further investigation and inspection of additional Raychem terminations and splices revealed that the problem may exist throughout the plant. The licensee estimates that 4,000 to 5,000 terminations and splices may be affected. The licensee is mobilizing a task force to inspect, evaluate and repair discrepant Raychem installations. The inspection is scheduled to start June 2, 1986.

The licensee estimates that this effort will not be complete until startup.

The licensee's repair, replacement and engineering review of items requiring environmental qualification (EQ) is continuing. Although the NRC EQ inspection team identified no significant deficiencies, further inspection is planned prior to restart.

6 One of the plants four essential batteries is inoperable and the licensee has determined that two of the batteries require replacement.

Miscellaneous IBEW Local 245 voted on June 1,1986, with 87% of the membership voting, not to accept the licensee's contract offer.

The strike affects maintenance, operations and chemistry and health physics personnel. Security personnel are not affected and remain onsite. The walkout was orderly and the transition was smooth.

Plant functions are being performed by professional and management personnel. The senior resident inspector, for a brief time, was denied access to the site but is currently onsite monitoring the licensee's activities.

On June 6,1986 the Ad Hoc Committee will present their findings to the Commission.

The licensee had appointed B. Carrick acting director of Nuclear Facility Engineering following the resignation of H. Brinkmari.

The licensee has appointed J. Fay acting general manager of Nuclear Engineering to replace 0. J. Mavro who is heading the Raychem task force.

Judge Hoyt has issued an order for a hearing to be held on June 24 and 25 in Sandusky, Ohio. The hearing involves NRC approval of the licensee's request to bury byproduct material onsite.
