SUBJECT:

AUDIT CPRT ON SAFETY SIGNIFICANT EVALUATION AND APPENDIX P ISSUES On February 19, 1986, the NRC staff and its consultants conducted an audit on approaches taken by the Comanche Peak Review Team (CPRT) for safety significant evaluation and for resolving concerns identified in Appendix P of the Comanche Peak SSER No. 11. The audit was held at the plant site in Glen Rose, Texas.

Persons who attended the audit are listed in the Enclosure. Major findings of the audit are as follows:

1. The Matrix System We were told by the CPRT that individual matrices will be established for all issues of concern. In each matrix, allegations and concerns from all sources and related to the same issue will be documented, including information from Appendix P, SSERs, and reports of SIT, CAT, SRT, CYGNA, Teledyne, Region IV, self-initiated efforts, etc. The matrices will be updated whenever new information becomes available, such as results of sample inspections.
2. Information Evaluation Collective evaluation of all information related to the same issue will be conducted by CPRT for drawing conclusions and for determining additional efforts. Closing of each issue will be documented in the final report.

We were told that matrices for issues in the Mechanical / Piping area are under preparation and need six more weeks for showing some results.

3. Safety Significant Evaluation Engineering evaluation will be cotiducted by CPRT for each issue associated with a matrix. If a deviation is found which does not conform with a licensing comitment, its safety significance will be assessed.

Redundancy of a system will not be counted in the safety significant evaluation.

4. Root Cause and Generic Implication If an issue is found by CPRT as a a safety significant deficiency, or an adverse trend is identified, root cause and generic implication of the issue will be investigated. In such case, additional effort by CPRT, such as sample expansion, will be conducted.

We found that approaches as stated above appear reasonable. However, we cautioned CPRT the following: (1) Design drawings should compare with as-built conditions for deviation investigation; (2) Piping and support interfaces should be considered for potential safety significant deficiencies; (3)

Possible adverse effects of nonsafety class system or component failure to safety essential systems and components, the so-called 2 to 1 effects, should be considered in the safety significant evalua ion.
