The following information is submitted in response to Inspection Report 86-05, which concerns the inspection conducted by Mr. J. L. Coley of your office on February 24-28, 1986, at Plant Hatch.

One apparent violation was identified and was applicable only to Hatch Unit 1.

VIOLATION:

" Technical Specification 6.8.1.c requires that written procedures be established, approved, implemented, and maintained covering surveillance activities of safety-related equipment.

Contrary to the above, acid etch examinations, performed to locate reactor pressure vessel longitudinal welds C-3-A, C-3-B, and C-3-C for inservice inspection (surveillance), were accomplished without an approved procedure.

This is a Severity Level V violation (Supplement I)."

RESPONSE TO VIOLATION:

Admission or denial of alleged violation: The violation occurred.

Reason for the violation:

After attempts to visually identify the exact location of the subject Hatch Unit 1 reactor pressure vessel longitudinal welds were unsuccessful,

acid etching of the welds was the viable alternative.

Due to the unexpected dif ficulty of visually locating the welds and due to the necessity of examining these welds during the current maintenance / refueling outage, an acid etch examination (GPC) performed was incorporating guidance from a

Nuclear Regulatory Commission Office of Inspection and Enforcement Region II - Saite 2900 May 12, 1986 Page Two RESPONSE TO VICLATION: (Continued) identification procedure (42SP-ENG-010-1 ), ASME Code,Section IX, QW-470 (Etching-Processes and Reagents), and recommendations from the Electric Power Research Institute (EPRI) NDE Center.

These vessel welds were required to be ultrasonically examined during the current outage in order to meet ASME Code,Section XI, requirements.

Since the acid etch process was implemented to obtain information to assist the ins weld, it was not in;erpreted as being a " surveillance,gector in locating a as discussed in the Technical Specifications, requiring a specific procedure.

Corrective steps which have been taken and the results achieved:

A formal procedure for acid etch exaEinatinn of carbon and low-alloy steel is presently being written.

In addition, a letter was written to the affected contractor inspection personnel identifying the requirement that examinations on safety-related equipment are to be performed using formal, approved procedures.

Corrective steps which will Le taken to hvotd further violations:

The development and incorporation of a

procedure will allow acid etch examinations to be performed on various materials 3 such as stainless steel, carbon, and low-alloy steel.

The procedure will be controlled by GPC, and acid etch examinations on sa fe ty-rela ted equipment will be performed in accordance with an approved procedure.

Date when full compliance will be achieved:

A procedure will be written, approved, and implementel prTor to the next need for perfomanCs of acid etch examinations of safety-related equipment.

The aforementioned letter, which addressed the requirement that formal, app'.tved procedures be adhered to in performing acid etch examinations on safety-related equipment, was presented to the a ffected contracter inspection personnel by their nanagement on April 24, 1986.
