STATUS OF PREVIOUS INSFECTION FINDINGS:

(Closed) Nonconformance (85-01, Item B.1): Test plans 566-1674 Revision A dated May 1, 1984 and 566-1674-1 Revision A dated May 1, 1984, had no documented objective evidence of approval by the same organization that performed the original review and approval. The NRC inspector reviewed one engineering report and one test report and determined that the revision of the above documents were reviewed and approved by the same organization that performed the original review and approval.

(Closed) Nonconformance (85-01, Item B.2): Test reports 58883-1 dated November 20, 1984 and 58883 dated August 24, 1984, documented instances where deviations to the prescribed requirements occurred during testing but no Notices of Deviation were written. The NRC inspector reviewed one EQ program that had documented instances where deviations to the prescribed requirements occurred during testing. The NRC inspector verified that Notice of Deviations were written for those instances.

(0 pen) Nenconformance (85-01, Item B.3): There was no documented procedure for the control of mixing the chemical spray solution used during design basis event testing and monitoring the solution's PH.

The NRC inspector took no action on this item during this inspection.

Technical Evaluation: The NRC inspector and Sandia consultant (NRC inspection team) performed a technical evaluation of four programs for qualification testing of safety-related electrical equipment.

The following table summarizes the programs examined including equipment type and types of documents examined.

The NRC inspection team reviewed the EQ process prescribed in each test plan and test procedure and reviewed test results where applicable.

Each of the four test programs and related engineering documents were examined for the following as applicable:

and used to meet the requirements of IEEE-STD-323/1974, b.

Equipment interfaces were addressed.

Test acceptance criteria were established as described in the c.

test specification or in the design engineering documents, such as calculations and engineering letters to meet the requirements of IEEE-STD-323/1974.

Same equipment was used for all phases of testing and represented a standard production item.

Environmental conditions were established and described (e.g.,

pressure and temperature profiles, and thermal aging factors were consistent with those outlined in the test specification or. test plan),

All prerequisites for the given tests as outlined in the test specification had been met.

Test equipment included a description of all materials, parts, and subcomponents.

Notice of Anomaly reports were properly documented.

Functional performance requirements met.

With respect to program 53454, an EQ test (temperature and humidity) had been performed for Washington Public Power Supply System on a damper motor for use in Washington Nuclear Power Station No. 2.

The test report was in draft approval cycle during this inpection there-fore only the test procedure was reviewed. The test report will be reviewed during a future inspection.

Program 57657 consisted of an extensive analysis for Borg Warner to determine activation energies for all components in Borg Warner operators. An aging program was established based on the results.

Synergistic effects were also discussed in detail.

Program 53309 covered an EQ test (LOCA) of a Borg Warner Pneumatic /

Hydraulic operator for use in River Bend No.1.

Program 53450 covered two seismic tests on Borg Warner modulating electro-hydraulic operators which had been performed prior to the LOCA test. The NRC inspection team reviewed two Notices of Deviation relating to structural failures as a result of the seismic tests.

The first failure was due to a broken mounting bracket that supported the accumulator. The broken bracket was removed and returned to Borg Warner for evaluation. The second failure was due to a broken bolt and two loose bolts which held the modified accumulator mounting bracket. When the bolt broke the modified mounting bracket corner punctured the operator cover. Testing was on l'old during this in-spection but it was anticipated that the seu.nic test would resume shortly followed by LOCA testing. The NRC inspection team will review test results of seismic and LOCA testing during a future inspection.

No nonconformances were noted during the evaluation of the above listed programs.

Quality Assurance Manual Review The NRC team leader reviewed the changes that were incorporated into Revision F of the QA manual. The NRC team leader determined that the changes were minor and did not change Wyle's QA program with respect to the requirements of Appendix B to 10 CFR Part 50.

Calibration of Test Equipment The NRC team leader selected four pieces of measuring and test equip-ment that was used during the LOCA testing for program 53309. The NRC team leader reviewed calibration records to verify (1) that a calibration interval was established, (2) a calibration procedure existed, (3) calibration source was identified, (4) a location was identified, (5) standards were traceable to hBS, and (6) equipment accuracy was identified.

No nonconformances were identified during this review.
