ENCLOSURE PLANT V0GTLE - UNIT 1 NRC DCCKET 50-424 OPERATING LICENSE NPF-68 NRC NOTICE OF VIOLATION 88-17-01 and GPC RESPONSE VIOLATION 50-424/88-17-01 "Technical Specification 6.7.1.a requires that written procedures be established, implemented and maintained covering activities delineated in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Contrary to the above, on April 26, 1988, an adequate procedure had not been established to cover the control room annunciator inverter alignment and operation.

Failure to provide an adequate procedure to prescribe the alignment and operation for the backup power source resulted in a loss of all control room annunciators for a period of 12 minutes following the loss of the normal power supply.

This is a Severity Level IV violation (Supplement I)."

RESPONSE TO VIOLATION 50-424/88-17-01 Admission or denial of alleged violation:

The violation occurred as stated.

Reason for the violation:

The violation occurred due to a procedural inadequacy. The non-1E inverter output breaker, internal to the QAR3 panel, was not included in a lineup as a specific item.

This breaker connects the backup power source to the annunciators.

Corrective steps which have been taken and the results achieved:

The corrective measures taken were as folicws:

1) Procedure 11432-1, Rev. 3T, "120V AC Non-1E Instrument Distribution System Alignment" was temporarily revised on May 6, 1988 to include i

six breakers associated with inverter panels INCQAR3 and 1NCQAR2.

This information was formally included in Rev. 4 of this procedure on May 30, 1988.

2)A check for other circuits providing annunciator power showed that these are the only panels serving the Control Room annunciators.

3) Operator aids diagramming the panels internal circuitry and associated circuit breakers were installed in the panels on May 6,

1988.

Descriptive labels were also installed in the panels by this date.

4) A defective static transfer switch was replaced on June 2,1988, which restores full capability of automatic transfer of annunciator power.

The aforementioned corrective measures should preclude a recurrence of this event.

Date when iall ~ compliance will be achieved:

Full compliance was achieved on May 6, 1988 with the revision Procedure 11432-1 and the installation of operator aids in the panels.
