The following information is in response to Inspection Report Nos. 50-321/86-04 and 50-366/86-04, dated March 20, 1986 The Report concerns the inspection conducted by Messrs.

Kuzc from February 10 to February 13, 1986 Deviation:
The licensee informed the NRC in its March 11, 1985 response to a Notice of Violation dated February 15, 1985, that the failure to complete the unmonitored release surveillances resulted from supervisors failing to utilize the Radiochemistry Department's Surveillance matrix to assure procedural compliance.
Corrective actions included periodic information review of records by management to ensure continuing performance.
Contrary to the above, licensee management failed to take adequate corrective action to ensure continuing performance when the 1985 fourth quarter records pertaining to unmonitored release surveillance data were found missing.
Response to deviation:
Corrective steps which have been taken and results achieved:
The required management review of the surveillance matrix was performed prior to the inspection.
It was noted during the review that the signoff for
" Releases via Unplanned Routes" was missing for the fourth quarter of 1985.
Subsequent investigation indicated that the surveillance had been accomplished, but the records were not to be found.
Additional samples were taken to cover the subject period.

The importance of maintaining an up-to-date matrix was emphasized to all Radiochemistry Department foremen.
Following the inspection, an additional management review of the matrix was conducted.
No other discrepancies were noted.
An expanded management review was initiated on March 1, 1986, to ensure that Radiochemistry Department supervisory personnel review the foreman's matrix for completeness at least once per month.
If any discrepancies are noted, the affected foreman will take imediate corrective action to ensure that surveillance activities are completed within the specified time intervals.
Corrective steps which will be taken to avoid further deviations:
No further actions are necessary to prevent recurrence of the deviation.
Date when full compliance was achieved:
Full compliance was documented on March b,
1986, when the surveillance test data were retaken to ensure adequate documentation.
