Appendix A of Regulatory Guide 1.33 lists the following activites under Administrative Procedures:
Authorities and Responsibilities for Safe Operation...
Shift and Relief Turnovers Detroit Edison Procedure POM 21.000.01, " Shift Operations and Control Room," delineates the shift turnover responsibilities of the Nuclear Shif t Supervisor (NSS), the Nuclear Assistant Shift Supervisor (NASS), and the Nuclear Station Operator (NSO).
Included in the responsiblilities is a review of each combination operation panel for off-normal conditions.
Contrary to the above, on February 2 and 3, 1986 the licensee failed to follow Procedure 21.000.01 in that for two shift turnovers and intervening shifts they failed to detect or I
recognize that the Active Seismic Monitor was in an off-normal condition as indicated by the presence of a lighted control room annunciator.
i Corrective Action Taken and Results Achieved j
As noted in the inspection report, after the alarmed condition was noted by the NRC Inspector, an operator was dispatched to the-relay room to determine if there was an actual problem wi;h the seismic monitor.
He determined that the system had not been reset.
He verified operability of the monitor and reset it which in turn cleared annunciator 6D2 " Seismic System Event / Trouble".
The three Contol Room Operators and one Nuclear Assistant Shif t Supervisor were counseled and subsequently were issued discipline for failing to note the alarmed condition.
Corrective Action to Avoid Further Violation Surveillance procedure 44.090.01 for the Active Seismic Monitoring System had been signed of f as completed on February 2,
1986.
This procedure did not include a step to verify that alarm 6D2 was clear af ter completion of the surveillance.

Functional Test Checklist (Attachment 1 of 44.090.01) has been revised to include this verification.

In addition, Standing Order 86-08 was issued requiring the Control Room Operator to know the status of all annunciators in an alarmed condition and to discuss this information with his relief and at the shift turnover briefing daily.
