., g,7 Thank you for sending us Amendment No. 21 to our Materials License.
When we sent our check to Ms. Glenda Jackson on August 11, 1987, we enclosed a letter which requested several additional changes other than the name change. We are enclosing a copy of the letter which we mailed to Ms. Jackson.
The additional changes were as follows:
Condition ll-The name of the Radiation Safety Officer should be changed from Frederick J. Shipko to Joseph N. Klingensmith, Condition 13-The IDE for the Coratomic C-101-P is no longer in effect.
The final report on the IDE was issued to thelFDA on February 18, 1983.
The FDA acknowledged termination ol the IDE on March 17, 1983.
We believe that Condition 13 could therefore bs deleted from the Materials License.

a Condition 14-We would appreciate a clarification as to why Condition 14 appears in the Materials License.
Condition 1-The company name was changed from Coratomic, Inc., to Biocontrol Technology, Inc. (You did make this change on Amendment 21. )
Would it be possible for you to reissue Amendment 21 with these additional changes?
