Diesel Romu Supply Fan 09/15/88 Rod Control 1-CP-CP-111 Replaced pole shaft assembly on reactor Train A/B trip breakers and bypass breakers per Reactor Trip NRC Bulletin 88-01.

Switchgear 09/15/88 Nuclecr 1-NI-NM-6691 Multiple component failures at amplifier Instrumentation Trait. B caused erroneous indication. Replaced PAM Nuclear failed components.

Instrumentation Channel 09/21/88 Rod Control 1-CP-CP-111 Charging springs on bypass breaker failed Train B to charge during surveillance test. Re-Reactor Trip placed spring charging limit switch.
