The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear

Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and observations by tae inspector. The inspection findings are as follows:

3. During this inspection certain of your activities, as described below and/or attached, were in violation of NRC requirements and are t,eing

cited. This form is a NOTICE OF VIOLATION, which is required to be posted in accordance with 10 CFR 19.11.

10 CFR 3*i.70(f) requires that a licensee conduct the weekly surveys for removable contamination of ali areas

where radiopharmaceuticals are routinely prepared for use, administered, or stored, as required by 10 CFR 35.70(e), so as to be able to detect contamination on each wipe sample of 2,000 disintegrations per minute.

Contrary to the above, as of August 26,1998, the licensee did not conduct weekly surveys for contamination in

areas where radiopharmaceuticals were routinely prepared for use, administered, or stored in such a manner so

as to be able to detect contamination on each wipe sample of 2,000 disintegrations per minute. Specifically, the

equipment used to analyze the wipe samples did not properly convert counts per minutes to disintegrations per

minute.
