, Gentlemen:

This letter is in response to a notice of violation contained in Inspection Report No. 70-754/88-01. The violation concerned the failure to hydrostatically test and to tag self-contained breathing apparatus (SCBA) cylinders as required by the site respiratory protection program manual.

All cylinders had been inspected and recorded in a log on a monthly basis.

However, the cylinders were marked after inspection with a date stamp which was sometimes difficult to read and was subject to smudging.

The failure to hydrostatically test the cylinders was due to oversight.

All cylinders were hydrostatically tested as of April 21, 1988. To prevent recurrence of the problem, the cylinders will be scheduled for retesting every-five years, and a tag will be placed on each unit noting the required test date. This will be completed by June 30, 1988. Additionally, a gummed paper tag is also nov placed on each cylinder following ecch monthly inspection.

With the hydrostatic testing of the cylinders and the adoption of the new tag, the respiratory program is now in compliance.
