Enclosed for your signature is Form NRC-591, "Safety Inspection." Please note the apparent violations which were observed during the August 24, 1988, inspection of your NRC General Licensed acti/ities authorized pursuant to 10 CFR 150.20, and which were reported to you at the conclusion of the inspection.
The inspection was an examination of the radiography activities conducted in Oklahoma under the reciprocal recognition of your State of Kansas License 21-R620-01, as they relate to radiation safety and to compliance with the Commission's rules and regulations, and the conditions of the license.
The inspection consisted of selective examinations of procedures and representative records, interviews of personnel, independent measurements, and observations by the inspector.
During this inspection certain of your activities were found not to be conducted in full compliance with NRC requirements.
These items are identified in the attached NRC Form 591 which you are required to sign.
By signing, you agree to correct the violations within 30 days.
Please keep the original for your records and return the remaining copies in the envelope provided.
