APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station Operating License: NPF-47 During an NRC inspection conducted on April 11-15, 1988, violations of NRC requirements were identified. The violations involved the failure to 'ollow procedures for daily and weekly reviews of radiation work permits and hilure to maintain procedures. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988),

the violations are listed below:

A. Technical Specification 6.8.1 requires written procedures to be established, implemented, and maintained covering activities referenced in Appendix A of Regulatory Guide 1.33-1978. Procedure RSP-0200, "Radiation Work Permits," requires, in Section 6.6, that radiation protection date radiological survey data daily and personnel will review radiation protection and up/ designee will review active radiation work foreman permits weekly.

Contrary to the above, on April 14, 1988, the NRC inspector determined that during the period September 1,1987, through January 1,1988, that radiation protection personnel dia not review and update radiological survey data daily and radiation protection foreman / designee did not review active radiation work permits weekl/.

This is a Severity Level V violation. (Supplement I)(458/8810-01)

8. Technical Specification 6.8.1 requires that written procedures be maintained covering activities referenced in Appendix A of Regulatory .

Guide 1.33-1978. Procedure RSP-0200, "Radiation Work Permits," requires '

that personnel initiate a radiation work permit (Attachment 1) and 1 radiation work permit addendum (Attachment 3).

Cont ary to the above, on April 14, 1988, the NRC inspector determined the licensee had revised the radiation work permit and radiation work permit l addendum in November 1987 without revising Attachments 1 and 3 to  !
