SUBJECT:
SINGLE FAILURES AND OTHER DEFICIENCIES NOTED IN CONTROL ROOM EMERGENCY VENTILATION SYSTEMS Enclosed is an Engineering Evaluation report on tingle failures and other deficiencies observed in control room emergency venti 16 tion systems. This study catalogs ten different problems reported in licensee event reports or inspection reports. Although control room ventilation is currently under review as generic issue 83, the AE00 study illustrates specific flaws that have been uncovered in spite of licensee activities in response to the Three Mile Island Action Plan item !!! D.3.4.
This issue is important because it involves a reduction in the expected safety margin associated with the emergency ventilation system.
Also, enclosed is a draft of a supplement to a previous information notice on this subject, IE IN 86-76. We recorrend that this supplement be sent to all licensees and construction permittees to alert them to the specific deficiencies already observed at a strable number of plants.
