Page 2 of 2 The purpose of this modification is to extend the period of performance of this Cooperative Agreement through September 1,1999, at no additional cost or obligation to the Government.
The extension is necessitated to ONLY allow for the completion of three on-campus research projects that were initiated in FY98 and will not be completed until FY99. No additional work will be authorized under this Agreement. All work shall be performed in accordance with ORISE's Appointment Offer letter dated July 17,1993 to Dr. Yong D. Song, and appointment letters dated August 3,1998 to Dr. Saeed Lari, and Dr. Tom Whitney, which are attached hereto and made apart of this Cooperative Agreement. Accordingly, the Agreement is modified as follows:
On page 1 of the " NOTICE OF COOPERATIVE AGREEMENT AWARD," block no. 3 entitled
" COOPERATIVE AGREEMENT PERIOD," the expiration date is extended from "10/31/98" to "9/01/99."
All other terms and conditions under this Cooperative Agreement remain unchanged.
A summary of obligations for this Cooperative Agreement, from award date through the date of this action is given below:
Total FY95 Obligation Amount: $200,000.00 Total FY96 Obligation Amount:
$75,000.00 Total FY97 Obligation Amount: $202,000.00 Total FY98 Obligation Amount: $200,000.00 Cumulative Total of NRC Obligations: $677,000.00 This modification does not obligate any FY99 funds.
