20555 Gentlement Soecial Report Limiting Condition for Operation (LCO) 3.7.9 of the Trojan Technical Specifications (TTS) requires submittal of a Special Report if a 3-hour fire barrier is nonfunctional for greater than seven days.

Fire barriers in the areas described below were declared inoperable on the dates indicated and remained inoperable for greater than seven days.

In each case the compensatory measures of TTS 3.7.9 (checking local e

smoke detectors and establishing an hourly fire patrol) were established as required.

1.

Fire Damper ho. 56, located on the Turbine Driven Auxiliary Feedwater Pump Room west wall (Turbine Building - 45 foot level),

was declared inoperable on August 12, 1988, when it was discovered that the damper was not installed in accordance with the manufacturer's instructions.

The damper was installed under a design modification completed in early 1979.

Conformance with the manufacturer's instructions is necessary to maintain the J

Underwriter's Laboratory (UL) Listing which is necessary to assure operability of the damper.

Repair of this damper is expected to be completed by December 30, 1988.

2.

Door No. 314, the Train Bay West roll-up door located on the Turbine Building 45 foot level, was declared inoperable on August 29, 1988.

The door's automatic closing device failed, and the door is in need of replacement as a result of being dropped.

It is expected that the door will be replaced and returned to functional status by October 15, 1988.

Door No. 156 for the South Centrifugal Charging Pump Room, located on the Auxiliary Building 25 foot level, was declared inoperable on August 22, 1988, because of a sticking latch mechanism and possible bent hinges.

Repair of this door is expected to be completed by September 30, 1988.

Door No. 359 to the Auxiliary Building 77 foot level Valve Gallery was declared inoperable on August 11, 1988 because of a sticking l

latch mechanism.

Repair is expected to be completed by September 30, 1988.

I 5.

Penetration Seal No. 3104, located on the 61 foot elevation of the Turbine Building east wall, was declared inoperable on August _19, 1988.

A conduit which was neither grouted nor foamed was found to be routed through the penetration, with both ends cut off.

A design change which affected this penetration had recently been worked; however, repair of this seal was overlooked.

The penetration seal was repaired and returned to service on September 1, 1988.

Internal event reports are being initiated to determine root cause and additional corrective actions for the situations described in item 1.
