Program Approval for Radioactive Material Packages No.
0416 expires on March 31, 1986 We are requesting renewal of this Q.A. Program Approval for Radioactive Material Packages No. 0416.
We have included a copy of the Quality Assurance Program with our change of corporation name and an up to date Radio-graphic Operations Organization Chart.
Our renewal request is accompanied by the $150.00 appli-cation fee.
We trust this will provide for continuation of the Q..A.
Program.
We are now an Agreement State License subject to the requirements of 10 CFR Part 71.
