NRC INSPECTION REPORT NO. 50-289/98-06 AND NOTICE OF VIOLATION

Dear Mr. Langenbach:

The NRC conducted an inspection between August 10 and August 27,1998, at the Three
Mile Island Nuclear Station. The inspection was focused on your resolution of the items
i
identified during the 1996/1997 NRC Architect Engineering (AE) Team inspection
!
(50-289/96-201).- At the conclusion of the inspection, the results were discussed with
'
you, Mr. Art Rone and others of your staff at an exit meeting held on August 27,1998.
!
Action taken to address AE team open items were found to be appropriate, msulting in
l
'
closure of those items. However, one item, involving a change made to the makeup
system cross-connect valves, was identified as a violation of NRC requirements. The
,
safety evaluation associated with the changes did not address the most limiting
configuration of the system, thereby resulting in a pressure-level correlation for the makeup
tank that was not sufficiently bounding. The violation is cited in the enclosed Notice of
'
Violation (Notice) because changing the configuration of the cross-connect valves created
the possibility of introducing gas (from the makeup tank) into the suction of makeup pump
P-1C, and as such, introduced an unreviewed safety question.
You are required to respond to this letter, and should follow the instructions specified in
the enclosed Notice when preparing your response. Your response should address the
justification for continued operation with the cross-connect valves open. The NRC will use
your response, in part, to determine whether further enforcement action is necessary to
,
ensure compliance with regulatory requirements.
