20555 Subj ec t: NPPD Response to Inspection Report 50-298/88-11 Gentlemen:

This letter is written in response to your letter dated May 12, 1988, transmitting Inspec tion Repor t 50-298/88-11. Therein you indicated that certain of our activities were in violation of NRC requirements.

Following is the statement of violation and our response in accordance with 10CFR2.201.

Statement of Violation Technical Specification 6.3, Procedures and Programs, s tates that written procedures and instructions, including applicable check-off lists, shall be established, implemented, and maintained for implementing procedures of the Emergency Plan.

Emergency Plan Implementing Procedure 5.7.21 states that "This procedure provides a means of ensuring readiness and availability of equipment required for the immediate ac tion s teps of all four emergency classification action levels."

Contrary to the above, the emergency telephone directory located in the Emergency Operations Facility and Operational Support Centers contained outdated information. Additionally, one emergency kit located at the Nemaha County Hospital was missing masking tape and a cardboard storage box.

This is a Severity Level V violation.

Reason For Violation The latest revision of the Emergency Telephone Directory (dated March 18, 1988) was transmitted to assigned personnel on March 18, 1988. The latest revision had not ye t been incorporated into the Emergency Telephone Direc tory located in the Maintenance Operational Support Center and the Chem /HP Operational Support Center at the time of the inspector's review. Due to the current outage activities the latest revision was not incorporated into the assigned directories in a timely manner. The directory in the Technical Support Center (not Emergency Operations Facility as stated in the violation) contained the entire p[3[

Nuclear Regulatory Commission May 25, 1988 Page 2 One emergency kit at the Nemaha County Hospital was missing several rolls of tape and a cardboard box.

A review of quarterly inventory checks revealed that all inventory was accounted for on February 2, 1988.

It is not known why the items were missing but this could be due to the current outage and personnel being sent to the hospital for medical treatment. The hospital staff may have removed the items for use on CNS injured personnel without informing NPPD personnel of the need to replenish these supplies.

Corrective Steps Which Have Been Taken and the Results Achieved Immediately upon being informed by the NRC inspector of the discrepancies found in the Emergency Telephone Directories the Emergency Preparedness Department sent a memo to all CNS directory holders requesting verification of their assigned directory.

Verification acknowledgements were returned to the EP Department.

The items missing from the hospital kits were replaced and the kits inventoried.

Corrective Steps Which Will Be Taken to Avoid Further Violations The Emergency Preparedness Staff will assume the responsibility of updating the Maintenance and Chemistry /HP OSC Emergency Telephone Directories in an effort to get the latest revisions incorporated in a timely manner.

The kits at the Nemaha County Hospital have been sealed with serialized seals.

Instructions have been placed on the kits for the hospital staf f to call the EP Department if the seals are found broken.
