NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on January 27 through February 24, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985),'the following violation was identified:

10 CFR 50, Appendix B, Criterion II states in part: The (quality assurance) program shall provide for indoctrination and training of personnel performing activities affecting' quality as necessary to assure that suitable proficiency is achieved and maintained.

The IP Operational Quality Assurance Manual, Chapter 2, Paragraph 2.2.4, states in part: Each Manager or Director is responsible for the proper qualification of assigned personnel performing activities related to CPS. This includes establishing and maintaining formal documented training programs to ensure that personnel performing activities affecting quality are appropriately trained in the principles and techniques of the activity being performed; are instructed as to purpose, scope, and implementation of governing documents; and that they maintain the required proficiency.

Contrary to the above, Illinois Power Company failed to assure that Stone and Webster (the Illinois Power Company maintenance contractor) personnel, working under the Illinois Power Company Quality Assurance Program and performing activities affecting quality, were properly trained and indoctrinated. On January 29, 1986, the resident inspector observed a Stone and Webster electrician installing a modification to the motor actuator for the high pressure core spray injection valve (Valve No. 1E22F004). The electrician had been employed at the Clinton site for approximately two weeks; he had never worked at a nuclear power' plant before his arrival at Clinton Power Station; and he had been neither trained nor indoctrinated in the Clinton Power Station quality program prior to being assigned to perform safety-related work.

, of 200 Stone and Webster maintenance crafts had not been trained or l indoctrinated prior to being released to perform work at Clinton.

l This is a Severity Level IV violation (Supplement II).

! explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid

, further violations; and (3) the date when full compliance will be achieved.
