GN-1456 Referer.ce: Vogtle Electric Generating Plant - Unit 2 USNRC Inspection P.eport 50-425/88-12 Georgia Power Company wishes to submit the following information concerning the violation identified in the referenced inspection report.
Violation 50-425/88-12 "Failure to Provide Adequate Instructions to Ensure the Proper Installation of Flow Elements and Orf fices" Georgia Power Company (GPC) acknowledges the violation as identified in the USNRC report.
The violation is attributed to personnel error resulting from inattention to detail and lack of specific procedural instruction for the installation of orifice plates and flow elements.
To determine the extent of the violation, all installed flow elements / flow orifices were inspected.
Of two hundred and eighty one (281) devices examined, fifteen (15) were noted with incorrect flow direction in addition to the three (3) cited by the USNRC inspection.
All eighteen (18) incorrectly installed flow devices have since been removed and reinstalled in the correct orientation.
To prevent recurrence, Pullman Procedure IX-5 (bolting installation) has been revised to more clearly specify the installation orientation and to require QC verification of that orientation.
Flow element installation has been limited to personnel in the Pullman Flush / Hydro group and those personnel have been trained in the revised procedural requirements.
Flow devices are now being installed to these requirements.
Full compliance was achieved on May 10, 1988.
