NRC letter to TVA dated December 18, 1986, "Generic Lotter 83-28, Items 3.1.1, 3.1.2, 3.2.1, 3.2.2, 4.1 and 4.5.1 for Sequoyah" This letter is to provide NRC a final response to GL 83-28, item 4.1, by providing the information requested in reference 2 concerning verification of the undervoltage trip attachments (UVTAs) at SQN for Westinghouse Electric Corporation (W) 08-50 reactor trip breakers (RTBs).

This information consisted of a visual inspection of DB-50 breakers to ensure that post-1972 UVTAs are installed and an inspection of the spare UVTA inventory was conducted before use to ensure that they also were of the post-1972 vintage.

TVA committed to perform the two aforementioned inspections in reference 1.

From discussions with maintenance personnel, a maintenance request (MR) was written to perform visual inspections of SQN's 08-50 breakers to ensure the UVTAs were of the post-1972 vintage.

However, this MR was subsequently not approved because it was decided that an MR was not necessary to perform the visual inspections.

Therefore, the documentation of the visual inspections and confirmation of the results are not available.

TVA does have documentation available to confirm that i

the UVTAs installed in the 08-50 RTBs and bypass breakers are of the post-1972 vintage.

However, this documentation is a result of the replacement of these UVTAs and not a result of the visual inspections comaitted to in reference 1.

The enclosure provides a listing of the l

work requests (hrs) used to perform the UVTA replacements with the l

verified post-1972 UVTAs.

, MAY 231988 U.S. Nu: lear Regulatory Commission TVA has also verified that the replacement UVTAs purchased from H on Purchase Requisition 84P64-343921 were of the type that incorporated the modifications specified in 14 document number NCD-ELEC-18 dated December 12, 1971. Not only were these modifications incorporated in the UVTAs, but some additional design features that superseded some of those recommended in NCD-ELEC-18 were also included.

The current stock of spare UVTAs is from the above contract and a later contract (85P64-362714).

Therefore, no pre-1972 UVTAs are available at SQN for issue or installation.

Based upon this information, TVA has verified that the UVTAs installed and those in stock are of the post-1972 vintage. This information should provide NRC the final documentation required by reference 2 and complete TVA's response for the Safety Issues Management System item numbers M53098 (unit 1) and H53099 (unit 2).
