Dear Dr. Paperiello:

The following report is being made that involves a condition discovered during the shipment of Model UX-30 packages. A trailer of five UX-30 packages was loaded with 30B UF cylinders 6

at the Portsmouth Gaseous Diffusion Plant (PORTS). On August 4,1998, the trailer was shipped to the GE Nuclear production facility in Wilmington, North Carolina. The UX-30's are owned by GE and are provided to PORTS for transporting UF,, to the GE facility. The contents of the involved shipment was low enriched (4.95%) uranium.

During a routine safety / fuel stop, the Tri-State Motor Transit (TSMT) driver reported he had i

noticed that several of the overpack locking pins were not in the container holes and were hanging by their lanyards. TSMT contacted GE and subsequently the driver was instructed on how to reinstall the pins. The driver reinserted the pins and completed the shipment without incident.

GE notified PORTS of the incident and UF shipments were placed on hold pending the results 6

of the investigation.

USEC's investigation into this incident revealed that a PORTS Traffic Department Inspector had visually checked the trailer to confirm that all overpack locking pins were installed before the shipment left PORTS. To provide greater assurance that pins are fully engaged and functioning as designed, USEC revised our shipping procedure to include additional physical checks to assure U

overpack ball lock pins are locked in position. The revised procedure was issued on September 9,1998.
