Dear Mr. Rakiec:
This letter is in response to your application transmitted in letters dated June 5,1998 and July 6, I
1998, requesting reactivation of the registration for the Model No.1001, Smoke Detector. We are in the process of evaluating your request. However, in order to continue our evaluation, the following information is necessary:
1.
In order for us to reactivate Registration Certificate NR-323-D-101-E, we need to ask you to confirm in your response to this letter that you have read, understood and committed to the statements that were made in the original application by Generation 2, Inc. Specifically, please state that you have read, understood, and committed to the statements in the application dated May 18,1990, and letters dated August 10,1990, and September 13,1990, with enclosures thereto. NRC had issued the Certificate on the basis of these documents and they are listed in the Registration Certificate; consequently, for reactivating it we also need a commitment to the statements in these documents.
2.
Please specify that the label on the smoke detectors will specify Hemisphere Forwarding, Inc. with its address in the United States instead of Generation 2, Inc.
3.
Please indicate whether the name or the address of the manufacturer has changed from that shown on Registration Certificate No. NR-323-D-101-E, dated September 26,1990.
Please provide the requested information within thirty (30) days. If we do not receive the requested information within thirty (30) days of the date of this letter, we will consider your applicat!an as having been abandoned by you.
