Dear Mr. Bmwn:
l In response to your telephone request on September 18,1998, please note that the modification of the operating handle attachment of the Model SH-F series source holder L
devices is acceptable for licensing purposes in accordance with the conditions of the l
registration certificate (KY-512-D-101-B).
Specifically, as requested, we have compared the welded ' attachment of the handle to the shutter (in Drawing Nos. D-55000 and D-57418 in your application dated February 12,1997) to j'
the screw-type attachment (in Drawing Nos. 'D-58779 and D-58751 in your application dated June 22,1998). We have also reviewed the information provided in your telephone communication on September 22,1998 and your letter dated September 23,1998. Based on this information, we have determined that the safety function of the device is not affected by the design change. You may fabricate the SH-F series source holder devices with the screw-type handle attachment, with the screws secured with a thread locking compound, prior to completion of NRC's review of your application, dated June 22,1998. However, please be advised that you must manufacture and distribute the product, with the exception of the i
attachment method of the handle, in accordance with the statements and representations contained in registration certificate above. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificate.
You are obligated to notify us promptly in writing should you decide to no longer manufacture or offer service support for the product.
