This refers to the inspection conducted by Mr. James T. Conway of this office on November 4-8, 1985, of your facility at Lynchburg, Virginia, and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.

This inspection was made as a result of the receipt of 10 CFR Part 50.55(e),

Licensee Event (10 CFR 50.73), and 10 CFR Part 21 reports from: (1) Gulf States Utilities Company relating to the loss of torque of Reliance motors containing magnesium rotors, (2) Washington Public Power Supply System pertaining to valve / actuator weight discrepancies, (3) Public Service Electric and Gas Company on replacement actuators having different spring packs, and (4) Connonwealth Edison and Tennessee Valley Authority on qualification of internal wiring. Within these areas the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.

With regard to the environmental qualification of internal wiring of your motor valve operators, the inspection determined that even though your operator qualification test reports do not specifically address internal wiring or wiring qualification, you are able to support (by separate documentation) qualification of internal wiring (specifically Raychem Flametrol and/or Rockbestos Firewall SIS) you have installed in nuclear supplied valve operators. The inspection also determined that while you can support qualification of the above wires you have insta11ad, valve manufacturers, licensees, or others may have added wires which are not qualified by your data. Since the inspection at Limitorque, the NR0'has further determined, from inspections at several licensees that there is some Limitorque installed wiring (e.g., TEW and TW) in valve operators used in nuclear plants in harsh environment, safety-related systems that is not qualified by the Raychem or Rockbestos data. Even though these operators and their wiring may have been manufactured several years ago to non-nuclear requirements, licensees have a requirement to qualify the operators and wiring to appropriate qualification requirements. qualified by the Raychem or Rockbestos data, the NRC is concerned with how this wiring is environmentally qualified for use in Limitorque operators used in nuclear plants.

February 28, 1986 During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Significant items included failures to: (1) maintain records to assure compliance with 10 CFR Part 21 requirements, (2) notify the NRC of a defect, (3) have appropriate documentation relating to indoctrination / training of personnel, (4) impose quality requirements on

{ suppliers / manufacturers, (5) have sufficient qualification records for

nondestructive examination personnel, and (6) follow procedural requirements l'

pertaining to the evaluation of defects. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to j submit to this office within 30 days from the date of this letter a written i statement containing: (1) a description of steps that have been or will be ,

I taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to '

1 extending your response time for good cause shown.

You are also requested to submit a similar written statement for each item which appears in the enclosed Notice of Nonconformance.

The responses requested by this letter are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, FL 96-511.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of f this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
