SUBJECT: NRC COMBINED INSPECTION 50-245/98-212; 50-336/98-212;

50-423/98-212

On August 17,1998, the NRC completed an inspection at Millstone Units 1,2 & 3 reactor

facilities. The enclosed report presents the results of that inspection.

During the 7-week period covered by this inspection report, your conduct of activities at

l

the Millstone facilities was generally characterized by safety-conscious operations, sound

j

engineering and maintenance practices.

l

l

As documented in the enclosed report, we focused our attention to Unit 3 operations

l

throughout the inspection period. Specifically, we conducted around-the-clock inspection

j

'in the control room from reactor criticality through thirty percent power. We noted

!

improved performance since the NRC OSTI inspection as characterized by coaservative

decision making and deliberate plant operation. Our observations are consistent with our

expectations for a plant returning to power following an outage over two years in duration.

l

During the startup, several procedure issues were identified in procedures which hadn't

l{

l

been used or reviewed since the shutdown, because operators followed your new, higher

-

l

standard for procedure compliance. Although operators appear to be properly addressing

l

these procedural issues as they arise, management attention to this area may be warranted

to prevent constant challenges to plant personnel. We will continue to assess your at-

power performance with a focus on safety and conservative decision making.

At Unit 2, we found that, despite the challenges that emergent work places on your

crganization, the quality of your efforts in addressing restart issues was generally good.

Three Significant items List issues involving specific technical areas were closed.

l

Additionally, substantial progress was noted in programmatic areas including the Material

l

Equipment and Parts List Program; the Electrical Equipment Qualification Program, the High

i

Energy Line Break Program, as well as your efforts to address surveillance procedure

quality issues.

l

Also at Unit 2, your thorough review and assessment of the circumstances surrounding the

l'

personnel contaminations that occurred.during radiological work performed inside

containment on May 23,1998, identified weaknesses with regard to the performance of

the Unit 2 radiation protection organization. Although weaknesses in the performance of

the Unit 2 radiological protection organization resulted in a violation of NRC regulatory

i

requirements, the coincident personnel exposures were not significant, and you initiated

j

i

comprehensive corrective actions.

in accordance with your letter per 10 CFR 50.82 to the NRC, dated July 21,1998, the

authorization under License No. DPR-21 to operate the Millstone Unit 1 reactor was

l

relinquished. The Unit 1 staff initiated plans to reduce and reorganize the plant staff for

!

the transition from an extended maintenance mode to a decommissioning organization.

'

While maintenance and operations activities were reduced, personnel continue to focus

their efforts on assuring safe storage of the fuelin the spent fuel pool, and continued

compliance with the operating license. Plans are also being formulated to address the issue

of interconnecting systems between all three Millstone units.

Consistent with the commission's direction to abolish the Special Projects Office, the

inspection function for Millstone Station has been returned to the Office of the Regional

Administrator, Region 1. Nevertheless, the NRC's Unit 2 inspection program continues to

be governed by the Restart Assessment Plan, and reutart of the unit must have commission

approval.
