Dear Mr. Martin:
This is pursuant to the reference license Section 5.1.3 which requires notification to your office of detections of leakage of process waste lagoon liners, including the results of the investigation and corrective action taken.
During activation of the between lagoon liners sampling systems on May 28, 1988 solution was obtained from sampling heads for Lagoons No. 3 and 4.
A breach of the upper liners was confirmed by subsequent sampler activations; the flow rate from the sampler heads averaged 10-20 gallons per day.
Lagoon 3 was constructed on top of the original Petromat lined lagoon and sample heads exist between the Petromat liner and the lower Hypalon liner.
Activation of these samplers did not produce solution indicating soundness of the lower liner.
Lagoon 4 is not so equipped.
Sampling of the wells adjacent to Lagoons No. 3 and 4 gave no indication of leakage to groundwater.
The Lagoon 3 inventory was reduced and several potential breaches of the upper liner around the peripheral berm were repaired.
The Lagoon 3 inventory was increased with additional waste solution with no subsequent leakage found between the liners.
The lagoon 3 upper liner is now believed to be sound.
Lagoon 4 has been emptied of solution and is currently being emptied of blow sand.
